摘　　要：目的:分析心脏外科手术患者术后发生中枢神经系统并发症的影响因素。方法:回顾性分析2021年1月至2023年5月于该院行心脏外科手术治疗的90例患者的临床资料,根据术后是否发生中枢神经系统并发症将其分为发生组和未发生组,分析心脏外科手术患者术后发生中枢神经系统并发症的影响因素。结果:90例心脏外科手术患者中,术后53例发生中枢神经系统并发症,占58.89%;两组性别、体质量指数、吸烟史、饮酒史、合并高血压、合并糖尿病、合并慢性阻塞性肺疾病、术中出血量、术中体外循环不稳定比较,差异均无统计学意义(P>0.05);两组年龄、卒中史、手术时间、术后低氧血症、围术期心房颤动比较,差异均有统计学意义(P<0.05);Logistic回归分析结果显示,年龄≥60岁、有卒中史、术后低氧血症、围术期心房颤动均为心脏外科手术患者术后发生中枢神经系统并发症的危险因素(OR>1,P<0.05)。结论:年龄≥60岁、有卒中史、术后低氧血症、围术期心房颤动均为心脏外科手术患者术后发生中枢神经系统并发症的危险因素。Objective:To analyze influencing factors of postoperative central nervous system complications in patients undergoing cardiac surgeries.Methods:The clinical data of 90 patients who underwent cardiac surgeries in this hospital from January 2021 to May 2023 were retrospectively analyzed.According to whether postoperative neurological complications occurred,they were divided into occurrence group and non-occurrence group.The influencing factors of postoperative central nervous system complications in the patients undergoing cardiac surgeries were analyzed.Results:Among the 90 cases of cardiac surgery patients,53 cases occurred postoperative central nervous system complications,accounting for 58.89%.There were no significant differences in gender,body mass index,smoking history,drinking history,combined hypertension,combined diabetes,combined chronic obstructive pulmonary disease,intraoperative blood loss,and intraoperative cardiopulmonary bypass instability between the two groups(P>0.05).There were significant differences in age,stroke history,operation time,postoperative hypoxemia and perioperative atrial fibrillation between the two groups(P<0.05).Logistic regression analysis showed that age≥60 years old,history of stroke,postoperative hypoxemia,and perioperative atrial fibrillation were risk factors for postoperative central nervous system complications in the patients undergoing cardiac surgeries(OR>1,P<0.05).Conclusions:Age≥60 years old,history of stroke,postoperative hypoxemia,and perioperative atrial fibrillation are the risk factors for postoperative central nervous system complications in the patients undergoing cardiac surgeries.
