炔雌醇环丙孕酮联合米非司酮治疗围绝经期异常子宫出血患者的效果

摘　　要：目的:观察炔雌醇环丙孕酮联合米非司酮治疗围绝经期异常子宫出血(AUB)患者的效果。方法:回顾性分析2021年8月至2023年5月该院收治的118例围绝经期AUB患者的临床资料,根据治疗方案不同将其分为观察组与对照组各59例。对照组采用米非司酮治疗,观察组在对照组基础上联合炔雌醇环丙孕酮片治疗。比较两组临床疗效,治疗前后疾病相关指标[月经期、月经量、子宫内膜厚度、血红蛋白(Hb)]、血清性激素[孕酮、卵泡刺激素(FSH)、雌二醇(E2)、黄体生成素(LH)]和血清血管生成素-1(Ang-1)、血管生成素-2(Ang-2)水平,以及不良反应发生率。结果:观察组治疗总有效率为96.61%(57/59),高于对照组的84.75%(50/59),差异有统计学意义(P<0.05);治疗后,两组月经期短于治疗前,且观察组短于对照组,两组月经量少于治疗前,且观察组少于对照组,两组子宫内膜厚度均小于治疗前,且观察组小于对照组,两组Hb水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);治疗3个月后,两组LH、FSH、E2、孕酮水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗3个月后,两组Ang-1和Ang-2水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:炔雌醇环丙孕酮联合米非司酮治疗围绝经期AUB患者可提高治疗总有效率,改善疾病相关指标水平,以及降低血清性激素、Ang-1和Ang-2水平,效果优于单纯使用米非司酮治疗。Objective:To observe effects of Ethinylestradiol and Cyproterone acetate combined with Mifepristone in treatment of patients with perimenopausal abnormal uterine bleeding(AUB).Methods:The clinical data of 118 patients with perimenopausal AUB admitted to this hospital from August 2021 to May 2023 were retrospectively analyzed.According to different treatment plans,they were divided into observation group and control group,59 cases in each group.The control group was treated with Mifepristone,while the observation group was treated with Ethinylestradiol and Cyproterone acetate tablets on the basis of that of the control group.The clinical efficacy,the levels of disease-related indexes[menstrual period,menstrual volume,endometrial thickness,hemoglobin(Hb)]levels,the serum sex hormone[progesterone,follicle stimulating hormone(FSH),estradiol(E2),luteinizing hormone(LH)],the serum angiopoietin-1(Ang-1)and the angiopoietin-2(Ang-2)before and after the treatment,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 96.61%(57/59),which was higher than 84.75%(50/59)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the menstrual period levels of the two groups were shorter than those before the treatment,and that in the observation group was shorter than that in the control group;the menstrual volume levels of the two groups were less than those before the treatment,and that in the observation group was less than that in the control group;the endometrial thickness levels of the two groups were smaller than those before the treatment,and that in the observation group was smaller than that in the control group;the Hb levels of the two groups were higher than those before the treatment,and that in the observation group was higher than that in the control group;and the differences were statistically significant(P<0.05).After 3 months of treatment,the levels of LH,FSH,E2 and progesterone in the two
