高频超声引导穿刺置管引流术与切开引流术治疗浅部肛周脓肿患者的效果比较

摘　　要：目的:比较高频超声引导穿刺置管引流术与切开引流术治疗浅部肛周脓肿患者的效果。方法:回顾性分析2021年3月至2024年3月该院收治的70例浅部肛周脓肿患者的临床资料,按照手术方法不同将其分为对照组和观察组各35例。对照组行切开引流术治疗,观察组行高频超声引导穿刺置管引流术治疗。比较两组临床疗效,围术期指标(手术时间、住院时间、术后下床活动时间、创面愈合时间)水平、手术前后疼痛[视觉模拟评分法(VAS)]评分,以及并发症发生率。结果:观察组治疗总有效率为94.29%(33/35),高于对照组的71.43%(25/35),差异有统计学意义(P<0.05);观察组手术时间、术后下床活动时间、创面愈合时间和住院时间均短于对照组,差异有统计学意义(P<0.05);术后7 d,两组VAS评分均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为5.71%(2/35),低于对照组的22.86%(8/35),差异有统计学意义(P<0.05)。结论:高频超声引导穿刺置管引流术治疗浅部肛周脓肿患者可提高治疗总有效率,改善围术期指标水平,降低疼痛评分和并发症发生率,效果优于切开引流术治疗。Objective:To compare effects of high-frequency ultrasound-guided percutaneous catheter drainage and incision and drainage in treatment of patients with superficial perianal abscess.Methods:The clinical data of 70 patients with superficial perianal abscess admitted to this hospital from March 2021 to March 2024 were retrospectively analyzed.According to different surgical methods,they were divided into control group and observation group,35 cases in each group.The control group was treated with incision and drainage,while the observation group was treated with high-frequency ultrasound guided percutaneous catheter drainage.The clinical efficacy,the perioperative indexes(operation time,hospitalization time,postoperative ambulation time,wound healing time)levels,the pain[visual analogue scale(VAS)]score before and after the surgery,and incidence of complications were compared between the two groups.Results:The total effective rate of the observation group was 94.29%(33/35),which was higher than 71.43%(25/35)of the control group,and the difference was statistically significant(P<0.05).The operation time,postoperative ambulation time,wound healing time and hospitalization time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Seven days after the surgery,the VAS scores of the two groups were lower than those before the surgery,that in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 5.71%(2/35),which was lower than 22.86%(8/35)in the control group,and the difference was statistically significant(P<0.05).Conclusions:High-frequency ultrasound guided percutaneous catheter drainage in the treatment of the patients with superficial perianal abscess can improve the total effective rate of treatment,improve the levels of perioperative indexes,and reduce the pain scores and the incidence of complications.Moreo
