摘 要:目的 研究胎儿生长受限(FGR)胎盘组织中沉默信息调节因子(SIRT)1的表达,探讨其在FGR中的意义。方法 选择于郑州市妇幼保健院2017年1月至2021年12月分娩的100例FGR胎盘组织作为FGR组,选择同期100例非FGR胎盘组织作为对照组,均不合并其他疾病或并发症。根据确诊FGR时间将FGR组分为早发型(35例)和晚发型(65例)。采用免疫组织化学法测定胎盘组织中SIRT1的表达。结果 FGR组分娩孕周、新生儿出生身长、新生儿出生体重、胎盘重量、Apgar 1 min评分均低于对照组(P<0.05)。SIRT1主要在细胞滋养细胞和合体滋养细胞核内表达,FGR组胎盘组织SIRT1阳性细胞比率、平均光密度值、高表达率均低于对照组(P<0.05)。早发型FGR胎盘组织SIRT1阳性细胞比率、平均光密度值、高表达率均低于晚发型(P<0.05),胎盘组织SIRT1表达与分娩孕周、新生儿出生身长、新生儿出生体重、胎盘重量呈正相关(P<0.05)。结论 SIRT1在FGR患者胎盘中低表达,SIRT1可能参与FGR的发病过程。Objective To study the expression of silence information regulator(SIRT)1 in fetal growth restraction(FGR)placental tissue,and to explore the significance in the occurrence of FGR.Methods A total of 100 cases of FGR placental tissue delivered from January 2017 to December 2021 in Zhengzhou Maternal and Child Health Hospital were selected as the FGR group,and 100 cases of non-FGR placental tissue were selected as the control group.They were not associated with other diseases or complications.FGR group was divided into early-onset FGR(35 cases)and late-onset FGR(65 cases)according to the time of diagnosis.Immunohistochemical method was used to determine the expression of SIRT1 in placenta.Results Gestational week,birth length,birth weight,placental weight and Apgar 1 minute score in FGR group were all lower than those in control group(P<0.05).SIRT1 was mainly expressed in cytotrophoblast cells and syncytiotrophoblast nuclei.The ratio of SIRT1 positive cells,average optical density and high expression rate of placental tissue in FGR group were lower than those in control group(P<0.05).The ratio of SIRT1 positive cells,average optical density value and high expression rate in placental tissues with early-onset FGR were lower than those with late-onset FGR(P<0.05).The expression of SIRT1 in placental tissues was positively correlated with gestational weeks,birth length,birth weight and placental weight of newborns(P<0.05).Conclusion SIRT1 is low expressed in placenta of FGR patients,and SIRT1 may be involved in the pathogenesis of FGR.
