腰方肌阻滞联合全身麻醉对经皮肾镜手术患者疼痛及快速康复效果的影响研究

摘　　要：目的:探究腰方肌阻滞联合全身麻醉对经皮肾镜手术患者的影响。方法:选取2022年11月—2024年2月于中山大学附属第三医院粤东医院进行经皮肾镜手术治疗的泌尿系结石患者80例作为研究对象,参照随机数字表法分为对照组和观察组,每组40例。对照组采用全身麻醉,观察组采用腰方肌阻滞联合全身麻醉。对比两组手术情况、疼痛情况、血流动力学指标。结果:观察组瑞芬太尼用量少于对照组,术后排气时间和住院时间短于对照组,差异有统计学意义(P<0.001)。术后6、12、24 h,观察组视觉模拟评分法评分低于对照组(P<0.001)。两组麻醉诱导前心率、平均动脉压水平比较,无统计学差异(P>0.05);碎石开始时、手术结束时,观察组心率、平均动脉压水平低于对照组(P<0.05)。结论:腰方肌阻滞联合全身麻醉应用于经皮肾镜手术,有助于减少瑞芬太尼用量,促进术后恢复,减轻术后疼痛。Objective:To explore the effect of lumboquadrate block combined with general anesthesia on percutaneous nephrolithotomy.Methods:A total of eighty patients with urinary calculus who underwent percutaneous nephroscopic surgery in Yuedong Hospital of the Third Affiliated Hospital of Sun Yat-sen University from November 2022 to February 2024 were selected as the study objects,and were divided into control group and observation group according to random number table method,with forty cases in each group.The control group was given general anesthesia,and the observation group was given lumboquadrate block combined with general anesthesia.Operation,pain and hemodynamic indexes were compared between the two groups.Results:The dosage of remifentanil in the observation group was less than that in the control group,and the postoperative exhaust time and hospital stay were shorter than that in the control group,the difference was statistically significant(P<0.001).6,12 and 24 h after operation,the visual analogue score of the observation group was lower than that of the control group(P<0.001).There were no significant differences in heart rate and mean arterial pressure before anesthesia induction between the two groups(P>0.05);at the beginning of lithotripsis and the end of operation,the heart rate and mean arterial pressure of observation group were lower than those of control group(P<0.05).Conclusion:Lumboquadrate block combined with general anesthesia in percutaneous nephrolithotomy is helpful to reduce the dosage of remifentanil,promote postoperative recovery and alleviate postoperative pain.
