尼可地尔联合硝酸甘油治疗非ST段抬高型急性冠脉综合征的效果

摘　　要：目的:研究尼可地尔联合硝酸甘油治疗非ST段抬高型急性冠脉综合征(NSTE-ACS)的临床价值。方法:选取2021年10月—2023年10月兰州大学第二医院收治的78例NSTE-ACS患者为研究对象,随机分为对照组和研究组,各39例。对照组采用硝酸甘油治疗,研究组采用尼可地尔联合硝酸甘油治疗。对比两组心电图ST段压低恢复正常时间、心绞痛发作缓解时间、血小板活化指标、缺血再灌注损伤相关指标。结果:研究组心电图ST段压低恢复正常时间、心绞痛发作缓解时间均短于对照组(P<0.001);治疗后,两组血小板α-颗粒膜蛋白、溶酶体膜蛋白、血小板聚集率水平降低,研究组低于对照组(P<0.05);治疗后4、24 h,两组超敏C反应蛋白、N末端B型利钠肽前体水平降低,研究组低于对照组(P<0.05)。结论:尼可地尔联合硝酸甘油治疗NSTE-ACS的效果较好,能够缓解心绞痛,改善血小板活化指标,减轻缺血再灌注损伤。Objective:To investigate the clinical value of nicodil combined with nitroglycerin in the treatment of non-ST segment elevation acute coronary syndrome(NSTE-ACS).Methods:A total of seventy-eight patients with NSTE-ACS admitted to Lanzhou University Second Hospital from October 2021 to October 2023 were selected as the study objects,and randomly divided into the control group and the study group,with thirty-nine cases each.The control group was treated with nitroglycerin,and the study group was treated with nicodil combined with nitroglycerin.The recovery time of ST segment depression,remission time of angina pectoris attack,platelet activation index and ischemia-reperfusion injury related index were compared between the two groups.Results:The normal time of ST segment depression and the remission time of angina pectoris in the study group were shorter than those in the control group(P<0.001);after treatment,the levels of plateletα-granular membrane protein,lysosomal membrane protein and platelet aggregation rate were decreased in two groups,and those in study group were lower than those in control group(P<0.05);at 4 and 24 h after treatment,the levels of hypersensitive C-reactive protein and N-terminal pro-brain natriuretic peptide in two groups were decreased,and those in the study group were lower than those in the control group(P<0.05).Conclusion:Nicodil combined with nitroglycerin in the treatment of NSTE-ACS has a better effect,which can relieve angina pectoris,improve platelet activation index,and alleviate ischemia reperfusion injury.
