耳石复位手法配合甲磺酸倍他司汀对良性阵发性位置性眩晕患者临床症状与预后结局的影响

摘　　要：目的:探讨耳石复位手法配合甲磺酸倍他司汀治疗对良性阵发性位置性眩晕(BPPV)患者临床症状与预后结局的影响。方法:选取2019年6月—2022年12月中新广州知识城医院收治的102例BPPV患者为研究对象,随机分为对照组和观察组,各51例。对照组应用甲磺酸倍他司汀治疗,观察组在对照组基础上配合耳石复位手法治疗。对比两组治疗效果、疾病复发率、平衡能力、前庭功能、血液流变学指标。结果:观察组治疗总有效率高于对照组(P=0.014);治疗后1、3、6个月,观察组疾病复发率低于对照组(P<0.05);治疗后,两组Berg平衡量表评分升高,前庭症状指数评分降低,观察组优于对照组(P<0.05);治疗后,两组全血低切黏度、全血高切黏度、血浆黏度、血浆纤维蛋白原水平降低,观察组低于对照组(P<0.05)。结论:耳石复位手法配合甲磺酸倍他司汀治疗BPPV的效果较好,能够改善临床症状,提高患者的平衡能力与前庭功能,恢复血液循环。Objective:To investigate the effect of otolith reduction manipulation combined with betahistine mesylate on the clinical symptoms and prognosis of benign paroxysmal positional vertigo(BPPV).Methods:A total of one hundred and two BPPV patients admitted to Zhongxin Guangzhou Zhishicheng Hospital from June 2019 to December 2022 were selected as the study objects,and randomly divided into control group and observation group,with fifty-one cases in each group.The control group was treated with betahistine mesylate,and the observation group was treated with otolith reduction manipulation on the basis of the control group.The therapeutic effect,recurrence rate,balance ability,vestibular function and hemorheology were compared between the two groups.Results:The total effective rate of observation group was higher than that of control group(P=0.014);at 1,3 and 6 months after treatment,the recurrence rate of the observation group was lower than that of the control group(P<0.05);after treatment,Berg Balance Scale score increased,vestibular symptom index score decreased,observation group was better than control group(P<0.05);after treatment,whole blood low tangential viscosity,whole blood high tangential viscosity,plasma viscosity and plasma fibrinogen levels were decreased in both groups,and the observation group was lower than the control group(P<0.05).Conclusion:Otolith reduction manipulation combined with betahistine mesylate has a good effect in the treatment of BPPV,which can improve clinical symptoms,improve patients'balance ability and vestibular function,and restore blood circulation.
