摘　　要：目的:分析一体化延伸护理对高血压患者服药依从性及血压控制情况的影响。方法:选取2023年1月—2024年5月贵州医科大学附属医院收治的高血压患者98例为研究对象,根据随机数字表法分为对照组和观察组,各49例。对照组应用常规护理,观察组在对照组基础上应用一体化延伸护理。比较两组服药依从性、自我管理评分、血压水平。结果:观察组服药总依从率高于对照组(P=0.025);护理后,两组运动锻炼、认知性症状管理实践、与医师沟通评分升高,观察组高于对照组(P<0.05);护理后,两组收缩压、舒张压水平降低,观察组低于对照组(P<0.05)。结论:一体化延伸护理能够有效实现院内与院外护理的衔接,积极提高患者的自我管理能力及服药依从性,使其血压得到有效控制。Objective:To analyze the effects of integrated extended care on medication adherence and blood pressure control in hypertensive patients.Methods:Ninety-eight hypertensive patients admitted to the Affiliated Hospital of Guizhou Medical University from January 2023 to May 2024 were selected as the study subjects,and were divided into a control group and an observation group of forty-nine cases each according to the random number table method.The control group applied conventional nursing,and the observation group applied integrated extended nursing on the basis of the control group.Compared medication adherence,self-management scores,and blood pressure levels between the two groups.Results:The total medication adherence rate of the observation group was higher than that of the control group(P=0.025);after the nursing,the scores of exercise,cognitive symptom management practice,and communication with physicians were elevated in the two groups,and the observation group was higher than that of the control group(P<0.05);after the nursing,the systolic blood pressure and diastolic blood pressure levels were reduced in the two groups,and the observation group was lower than that of the control group(P<0.05).Conclusion:Integrated extended care can effectively realize the connection between in-hospital and out-of-hospital care,actively improve patients'self-management ability and medication adherence,so that their blood pressure can be effectively controlled.
