摘　　要：目的:分析改良Devine术联合包皮远端皮瓣转移治疗小儿重度隐匿性阴茎的效果。方法:选取2021年3月—2023年12月阿克苏地区第一人民医院小儿外科收治的隐匿性阴茎患儿88例作为研究对象,通过随机数字表法分为对照组与观察组,各44例。对照组采用改良Devine术治疗,观察组采用改良Devine术联合包皮远端皮瓣转移治疗。比较两组疗效、手术相关指标及阴茎延长长度、术后并发症发生情况、生活质量评分。结果:观察组治疗总有效率高于对照组(P=0.044)。观察组手术时间、水肿消失时间短于对照组,阴茎延长长度大于对照组(P<0.05);两组术中出血量比较,差异无统计学意义(P>0.05)。观察组并发症总发生率低于对照组(P=0.024)。治疗后,两组生理机能、躯体质量、活力状况、情绪功能评分均升高,观察组高于对照组(P<0.05)。结论:改良Devine术联合包皮远端皮瓣转移治疗小儿重度隐匿性阴茎的效果显著,可促进水肿消失,延长阴茎长度,减少术后并发症发生,提高生活质量。Objective:To analyze the effect of modified Devine operation combined with distal prepuce flap transfer in the treatment of severe concealed penis in children.Methods:A total of eighty-eight children with concealed penis admitted to the pediatric surgery department of Aksu Prefecture First People's Hospital from March 2021 to December 2023 were selected as the study objects,and were divided into control group and observation group by random number table method,with forty-four cases in each group.The control group was treated with modified Devine,and the observation group was treated with modified Devine combined with distal prepuce flap transfer.The results of the two groups were compared,as well as the relative indexes of operation,the length of penis extension,the incidence of postoperative complications and the score of quality of life.Results:The total effective rate of observation group was higher than that of control group(P=0.044).The operation time and edema disappearance time of the observation group were shorter than that of the control group,and the penis lengthening length was longer than that of the control group(P<0.05);there was no significant difference in intraoperative blood loss between the two groups(P>0.05).The total complication rate of observation group was lower than that of control group(P=0.024).After treatment,the scores of physiological function,body mass,vitality and emotional function of both groups were increased,and the observation group was higher than the control group(P<0.05).Conclusion:Modified Devine combined with distal flap transfer of prepuce is effective in the treatment of severe concealed penis in children,which can promote the disappearance of edema,prolong the length of penis,reduce postoperative complications and improve the quality of life.
