超声引导冠突法神经阻滞联合布比卡因局部麻醉在三叉神经痛患者中的应用效果分析

摘　　要：目的:分析超声引导冠突法神经阻滞联合布比卡因局部麻醉在三叉神经痛患者中的应用效果。方法:选取2022年3月—2023年12月在北京市怀柔区中医医院接受治疗的三叉神经痛患者86例,通过双色球法随机分为观察组(超声引导冠突法神经阻滞联合布比卡因局部麻醉)与对照组(常规神经阻滞疗法),每组43例。比较两组疼痛程度与睡眠情况、不同时间镇静评分及不良事件发生率。结果:治疗后,两组视觉模拟评分法、睡眠质量评分表评分均降低,观察组低于对照组(P<0.05);术后2、4、12、24 h,两组Ramsay评分比较,差异均无统计学意义(P>0.05);观察组不良事件总发生率低于对照组(P=0.049)。结论:超声引导冠突法神经阻滞结合布比卡因局部麻醉能缓解三叉神经痛患者的疼痛症状,减少不良反应发生,改善睡眠状况,且能实现深度镇静效果。Objective:To analyze the effect of ultrasound-guided coronary nerve block combined with bupivacaine local anesthesia in patients with trigeminal neuralgia.Methods:A total of eighty-six patients with trigeminal neuralgia who received treatment in Beijing Huairou District Hospital of Traditional Chinese Medicine from March 2022 to December 2023 were selected and randomly divided into observation group(ultrasound-guided coronary nerve block combined with bupivacaine local anesthesia)and control group(conventional nerve block therapy)by bicolor ball method,with forty-three cases in each group.The pain degree,sleep condition,sedation score at different time and incidence of adverse events were compared between the two groups.Results:After treatment,the scores of visual analog scale and sleep quality scale were decreased in both groups,and the observation group was lower than the control group(P<0.05);at 2,4,12 and 24 h after surgery,there was no significant difference in Ramsay score between the two groups(P>0.05);the total incidence of adverse events in observation group was lower than that in control group(P=0.049).Conclusion:Ultrasound-guided coronary nerve block combined with bupivacaine local anesthesia can relieve the pain symptoms of patients with trigeminal neuralgia,reduce the occurrence of adverse reactions,improve sleep status,and achieve deep sedation effect.
