摘  要:目的研究儿童肺炎支原体肺炎(Mycoplasma pneumoniae pneumonia,MPP)病程中肺炎支原体肺炎(Mycoplasma pneumoniae,MP)核糖核酸(ribonucleic acid,RNA)、白细胞介素(interleukin,IL)17A、IL-12p70的水平变化及意义。方法选择山西省儿童医院2022年1月至2024年5月期间收治的149例肺炎患儿,将其分为非MPP组(64例,MP培养阴性)与MPP组(85例,MP培养阳性),同时检测MPP患儿MP RNA。所有MPP患儿采用常规方案治疗28 d,分析不同治疗时间点(治疗前、7 d、14 d、21 d、28 d)的MP RNA阳性率及血清IL-17A、IL-12p70水平变化,比较临床疗效不同的MPP患儿MP RNA阳性率及血清IL-17A、IL-12p70水平。结果MPP组血清IL-17A、IL-12p70水平显著高于非MPP组(P<0.05)。85例MPP患儿MP-RNA阳性率78.82%(67/85)。MPP组MP RNA阳性率及血清IL-17A、IL-12p70水平随MPP患儿治疗时间的延长而逐步下降(P均<0.05)。MPP治疗无效组MP RNA阳性率及血清IL-17A、IL-12p70水平显著高于治疗有效组(P均<0.05)。结论肺炎患儿中,MPP患儿血清IL-17A、IL-12p70水平明显更高,且治疗后血清IL-17A、IL-12p70水平逐渐下降,与MPP疗效关系密切。Objective To explore the changes and significance of Mycoplasma pneumoniae ribonucleic acid(RNA),06-17A,and IL-12p70 levels in children with Mycoplasma pneumoniae pneumonia.Methods A total of 149 hospitalized children with pneumonia in Shanxi Childen’s Hospital from January 2022 to May 2024 were selected and divided into non MPP group(64 cases,MP culture negative)and the MPP group(85 cases,MP culture positive).The MP RNA detection results of 149 children with pneumonia were compared with MP culture results,and the serum levels of IL-17A and IL-12p70 in the MPP and non-MPP groups were detected and compared.All children with MPP were treated with a conventional regimen for 28 d The MP RNA positive detection rate and serum IL-17A,IL-12p70 levels at different treatment time points(before treatment,7 d,14 d,21 d,28 d)were analyzed.The MP RNA positive detection rate and serum IL-17A and IL-12p70 levels in children with MPP with different clinical efficacy were compared.Results Among 149 children with pneumonia,85 cases were clinically diagnosed as MPP(MP Culture).When MP RNA was used to detect Mycoplasma pneumonia,the positive detection rate was 89.41%(76/85).The serum levels of IL-17A and IL-12p70 in the MPP group were significantly higher than those in non-MPP group(P<0.05).The positive detection rate of MP RNA and serum IL-17A and IL-12p70 levels gradually decreased with the extension of treatment time in children with MPP(P<0.05).The positive detection rate of MP RNA and the levels of serum IL-17A and IL-12p70 in the ineffective group were significantly higher than those in the effective group(P<0.05).Conclusion In children with pneumonia,the serum levels of IL-17A and IL-12p70 in children with MPP were significantly higher,and the serum levels of IL-17A and IL-12p70 gradually decreased after treatment,which was closely related to the curative effect of MPP.
