摘　　要：针对胃癌腹膜转移(peritoneal metastasis,PM)的治疗在不断发展。各种腹腔内化疗给药方式可通过血浆-腹膜屏障到达全身化疗无法覆盖的PM部位,其中细胞减灭术联合腹腔热灌注化疗应用最广泛,仍是目前针对胃癌PM最有效的治疗手段。腹腔内化疗在预防性、新辅助、辅助和姑息性治疗4个模式中均显示出良好的临床疗效。根据对胃癌PM病人的一般情况和PM严重程度的评估,选择合理的腹腔内治疗与系统治疗结合方式可能为这些病人提供个体化治疗。此文对不同模式下腹腔内化疗的研究进展、临床适应证和发展前景进行了综述。Targeted treatment of patients with peritoneal metastasis(PM)from gastric cancer(GC)has constantly evolved.Various delivery modes of intraperitoneal chemotherapy(IPC)may pass through plasma-peritoneal barrier to reach PM sites not accessible by systemic chemotherapy.And cytoreductive surgery plus hyperthermic IPC is currently the most effective treatment for PM.IPC has demonstrated some promising clinical efficacies in four different treatment modalities of prophylactic,neoadjuvant,adjuvant and palliative.It is now possible to select reasonable types of intraperitoneal and systemic treatments according to general conditions and peritoneal disease burden,thus providing individualized treatments.This reviews summarized the latest researches,clinical indications and development prospects of intraperitoneal IPC in different treatment modalities.
