摘　　要：目的:探讨心血管手术拟输血患者意外抗体分布及输血疗效。方法:选择2020年4月至2024年4月本院行心血管手术的患者1260例,对比输血治疗的疗效。结果:1260例住院患者血清经不规则抗体阳性有38例(3.02%)。输血无效组不规则抗体阳性率(23.15%)显著大于输血有效组(1.13%)(P<0.05)。不规则抗体阳性是心血管手术患者输血无效的独立危险因素(OR=3.974,P<0.05)。结论:对于心血管手术患者而言,尤其是针对女性、有多次妊娠史及输血史的患者,需加强对Rh、MNSs、Lewis等血型系统的检测力度,以提升对不规则抗体的识别率。Objective:to explore the distribution of accidental antibodies and the efficacy of blood transfusion in patients with cardiovascular surgery intended for blood transfusion.Methods:1260 patients who underwent cardiovascular surgery in our hospital from April 2020 to April 2024 were selected to compare the efficacy of blood transfusion therapy.Results:Out of 1260 hospitalized patients,38(3.02%)tested positive for irregular antibodies in their serum.The positive rate of irregular antibodies in the ineffective transfusion group(23.15%)was significantly higher than that in the effective transfusion group(1.13%)(P<0.05).Irregular antibody positivity is an independent risk factor for ineffective blood transfusion in patients undergoing cardiovascular surgery(OR=3.974,P<0.05).Conclusion:For patients undergoing cardiovascular surgery,especially for females with multiple pregnancies and transfusion histories,it is necessary to strengthen the testing of Rh,MNSs,Lewis and other blood type systems to improve the recognition rate of irregular antibodies.
