柴胡加龙骨牡蛎汤治疗抑郁症的作用机制分析及疗效系统评价

摘　　要：抑郁症属于中医“郁证”范畴,以肝郁为病机。柴胡加龙骨牡蛎汤为柴胡类方,具有疏肝解郁、安神定惊的功效,适用于治疗抑郁症且疗效好。通过收集柴胡加龙骨牡蛎汤治疗抑郁症的临床研究及实验研究文献,结合网络药理学研究分析其作用机制,发现柴胡加龙骨牡蛎汤发挥抗抑郁作用机制与中枢海马脑源性神经营养因子信号通路、环磷酸腺苷通路及丝裂原活化蛋白激酶通路等有关,并对柴胡加龙骨牡蛎汤治疗抑郁症的临床对照研究进行Meta分析,评估其疗效。结果提示柴胡加龙骨牡蛎汤或柴胡加龙骨牡蛎汤联合抗抑郁药物在抑郁症患者治疗效果[OR=2.68,95%CI(1.93,3.72)]及降低患者汉密尔顿抑郁量表评分[MD=1.19,95%CI(-2.21,4.58)]方面均优于单纯使用抗抑郁药物治疗。Depression belongs to the category of"depression syndrome"in traditional Chinese medicine,with liver depression as the pathogenesis.Chaihu Longgu Muli Decoction is a kind of Chaihu formula,which has the effect of soothing the liver,relieving depression,calming nerves and calming convulsions.It is suitable for the treatment of depression and has a good effect.This article collected the clinical and experimental research literature of Chaihu Longgu Muli Decoction in the treatment of depression,combined with network pharmacology research to analyze its mechanism of action.It found that Chaihu Longgu Muli Decoction plays an antidepressant role by participating in the brain derived neurotrophic factor signaling pathway,cyclic adenosine phosphate pathway and mitogen activated protein kinase pathway in the central hippocampus.Meta-analysis was carried out on the clinical control study of Chaihu Longgu Muli Decoction in the treatment of depression,and its efficacy was evaluated.The results showed that the treatment effect of Chaihu Longgu Muli Decoction or Chaihu Longgu Muli Decoction combined with antidepressants in patients with depression[OR=2.68,95%CI(1.93,3.72)]and the reduction of HAMD score of patients[MD=1.19,95%CI(-2.21,4.58)]were superior to the treatment of antidepressants alone.
