Nice结固定手术治疗第5跖骨Ⅰ区骨折的疗效

摘　　要：目的探讨Nice结固定手术治疗第5跖骨Ⅰ区骨折的临床效果。方法回顾性分析本院2014年3月-2021年6月收治的32例第5跖骨Ⅰ区骨折患者的病例资料,采用Nice结固定骨折手术治疗,分别记录视觉疼痛模拟评分(Visual analogue scale,VAS)、美国足踝协会(American orthopedic foot anile society,AOFAS)功能评分进行分析评估。结果32例患者均获得6~14个月随访,31例术口一期愈合,拆线时间为(17.44±1.81)d,1例术后第10天拆线后出现切口开裂渗液,经继续换药2周后愈合。切口平均长度为(36.22±3.90)mm,手术时间为(31.41±5.89)min,术中透视次数为(2.47±0.72)次。骨折复位满意度:优26例,良6例,优良率为100%,恢复完全负重时间为(11.97±1.99)周。术后VAS评分较术前明显改善(F=261.261,P<0.001);末次随访AOFAS评分为(92.09±2.56)分,功能恢复良好。结论Nice结固定方式治疗第5跖骨Ⅰ区骨折简单有效,临床疗效满意。Objective To evaluate the results of the Nice knot fixation technique for the treatment of zoneⅠfractures of the fifth metatarsal.Methods We retrospectively analyzed the case data of 32 patients with the fifth metatarsal regionⅠfracture who were admitted to our hospital from March 2014 to June 2021 and were surgically treated with Nice knot fixation of the fracture.We collected preoperative and postoperative data of the patients and recorded the visual analogue scale(VAS)and American orthopedic foot anile society(AOFAS)functional scores for analysis and evaluation,respectively.Results All 32 patients were followed up for 6-14 months.31 patients had stageⅠincisional healing,and the incisional healing and suture removal time was(17.44±1.81)d.One patient had incisional dehiscence and oozing after suture removal on postoperative day 10,and the incision healed in 2 weeks with continuous dressing changes.The mean incision length was(36.22±3.90)mm,the operative time was(31.41±5.89)min,and the number of intraoperative fluoroscopies was(2.47±0.72)times.Satisfaction with fracture reduction:26 cases were excellent,6 cases were good,the excellent and good rate was 100%,and the time to return to full weight-bearing was(11.97±1.99)weeks.The postoperative VAS score was significantly improved compared to the preoperative one(F=261.261,P<0.001);The AOFAS score at the last follow-up was(92.09±2.56),with good functional recovery.Conclusion Nice knot fixation is simple and effective in the treatment of fifth metatarsal zoneⅠfractures with satisfactory clinical results.
