Wiltse肌间隙入路与经皮椎弓根螺钉内固定术对胸腰椎骨折患者的治疗效果比较研究

摘　　要：比较Wiltse肌间隙入路椎弓根螺钉内固定术与经皮椎弓根螺钉内固定术对胸腰椎骨折患者的治疗效果。选取2021年1月—2022年11月于宁国市中医院骨科行椎弓根螺钉内固定术治疗的46例胸腰椎骨折患者为研究对象。随机将患者分为肌间隙入路组和经皮微创组,每组各23例,比较两组患者的治疗效果。结果显示,肌间隙入路组手术时间短于经皮微创组,术中出血量多于经皮微创组,术中透视次数少于经皮微创组,术后住院时间长于经皮微创组(P<0.05)。术后3个月,经皮微创组的VAS评分、ODI指数均低于肌间隙入路组(P<0.05)。研究发现,Wiltse肌间隙入路椎弓根螺钉内固定术治疗胸腰椎骨折患者的效果明显,有利于缩短患者的手术时间,减少术中透视次数,减轻机体应激反应和术后疼痛,促进患者胸腰椎功能的恢复。Compare the therapeutic effects of Wiltse intermuscular space approach pedicle screw fixation and percutaneous pedicle screw fixation on patients with thoracolumbar fractures.Forty six patients with thoracolumbar fractures who underwent pedicle screw internal fixation surgery in the Orthopedics Department of Ningguo Traditional Chinese Medicine Hospital from January 2021 to November 2022 were selected as the research subjects.Randomly divide the patients into a muscle gap access group and a percutaneous minimally invasive group,with 23 cases in each group,and compare the treatment effects of the two groups of patients.The results showed that the surgery time of the intermuscular space approach group was shorter than that of the percutaneous minimally invasive group,the intraoperative blood output was greater than that of the percutaneous minimally invasive group,the intraoperative fluoroscopy frequency was less than that of the percutaneous minimally invasive group,and the postoperative hospitalization time was longer than that of the percutaneous minimally invasive group(P<0.05).Three months after surgery,the VAS and ODI scores of the percutaneous minimally invasive group were lower than those of the intermuscular approach group(P<0.05).Research has found that the Wiltse intermuscular space approach for pedicle screw fixation is effective in treating patients with thoracolumbar fractures.It is beneficial for shortening the patient’s surgery time,reducing intraoperative fluoroscopy frequency,alleviating the body’s stress response and postoperative pain,and promoting the recovery of lumbar spine function.
