摘　　要：目的探讨双股钢丝“8”字捆绑克氏针张力带(即双结克氏针张力带)固定治疗髌骨骨折的效果。方法采用双结克氏针张力带固定治疗50例髌骨骨折患者。记录手术情况、骨折愈合时间、并发症发生情况。采用Lysholm评分标准评价膝关节功能。结果患者均获得随访,时间6~15(8.6±5.2)个月。手术时间45~95(64.3±16.7)min,术中出血量10~45(26.5±9.6)ml,术后1 d疼痛VAS评分2~6(3.3±1.0)分。骨折均完全愈合,时间10~16(12.8±3.0)周。术后发生内固定松动、退针2例,创伤性关节炎1例,软组织激惹2例,并发症发生率10%。术后6个月,采用Lysholm评分标准评价膝关节功能:优33例,良15例,可2例,优良率96%。结论双结克氏针张力带治疗髌骨骨折应力均匀分布,固定牢靠,内固定并发症少,疗效良好。Objective To investigate the effect of tension band fixation with double-strand wire 8-shaped binding Kirschner wire(It′s a double-knot Kirschner wire tension band)on the patellar fractures.Methods Fifty patients with patellar fractures were treated by tension band fixation with double-knot Kirschner wire tension band.The operation condition,fracture healing time and complications were recorded,and the knee joint function was evaluated by Lysholm score criterion.Results All patients were followed up for 6~15(8.6±5.2)months.The operation time was 45~95(64.3±16.7)min,the intraoperative blood loss was 10~45(26.5±9.6)ml,and the pain VAS was 2~6(3.3±1.0)points at 1 d postoperation.All the fractures healed completely,and the healing time was 10~16(12.8±3.0)weeks.There were 2 case of internal fixation loosening and needle withdrawal,1 case of traumatic arthritis,and 2 cases of soft tissue irritation,with a complication incidence rate of 10%.At 6 months after operation,Lysholm score was used to evaluate the knee joint function:33 cases were excellent,15 good,and 2 fair,with an excellent-good rate of 96%.Conclusions The double-knot Kirschner wire tension band in the treatment of patellar fracture has the advantages of uniform distribution of stress,reliable fixation,few complications of internal fixation,with good effect.
