摘 要:目的分析甲状腺激素和同型半胱氨酸(Homocysteine,HCY)与女性不孕的相关性。方法选择2022年6月—2023年9月本院收治的60例不孕女性患者作为观察组,将同期在本院进行健康体检的已生育健康女性60名作为对照组,使用全自动生化仪和电化学发光免疫分析仪,对两组研究对象甲状腺激素和Hcy水平进行检测,分析以上指标与女性不孕的相关性。结果观察组FT3、FT4水平均明显低于对照组,TSH、Hcy水平均明显高于对照组,差异有统计学意义(P<0.05)。甲状腺激素指标中,FT3、FT4与不孕呈负相关关系,TSH与不孕呈正相关关系(P<0.05)。Hcy水平与不孕呈正相关关系(P<0.05)。ROC曲线分析显示,FT3、FT4、TSH以及Hcy对女性不孕均有着较高的评估价值,AUC均>0.70。结论甲状腺激素和Hcy与女性不孕有着一定的相关性,其中FT3、FT4指标与女性不孕为负相关关系,TSH、Hcy与女性不孕为正相关关系,因此临床诊疗时应重点关注患者相关指标的变化,根据患者实际情况制定针对性的治疗方案。Objective To analyze the correlation between thyroid hormone and homocysteine(HCY)and female infertility.Methods 60 cases of infertile female patients admitted to our hospital from June 2022 to September 2023 were selected as the observation group,and 60 healthy females who had given birth and underwent health checkups at our hospital during the same period were selected as the control group.The levels of thyroid hormones and Hcy in both groups were measured using a fully automated biochemical analyzer and an electrochemiluminescence immunoanalyzer.The correlation between these indicators and female infertility was analyzed.Results The levels of FT3 and FT4 in the observation group were significantly lower than those in the control group,while the levels of TSH and Hcy were significantly higher,with statistically significant differences(P<0.05).Among the thyroid hormone indicators,FT3 and FT4 showed a negative correlation with infertility,whereas TSH showed a positive correlation with infertility(P<0.05).The level of Hcy also showed a positive correlation with infertility(P<0.05).ROC curve analysis indicated that FT3,FT4,TSH,and Hcy all had a high evaluative value for female infertility,with AUCs all greater than 0.70.Conclusions Thyroid hormones and Hcy have a certain correlation with female infertility,where FT3 and FT4 are negatively correlated with female infertility,and TSH and Hcy are positively correlated with female infertility.Therefore,in clinical diagnosis and treatment,attention should be paid to the changes in these indicators,and targeted treatment plans should be formulated based on the patient's actual situation.
