补中益气 益肾养肺汤联合常规西药治疗肺肾气虚型慢性肾小球肾炎的临床研究

摘　　要：目的探讨补中益气、益肾养肺汤联合常规西药治疗肺肾气虚型慢性肾小球肾炎(CGN)的临床效果。方法选取2020年4月至2021年9月浙江省温州市中医院收治的102例肺肾气虚型CGN患者,按治疗方法分为对照组和联合组各51例。对照组行常规西药治疗,联合组行补中益气、益肾养肺汤联用常规西药治疗。治疗前后,对比2组中医症状积分、肾功能指标[血清胱抑素C(CysC)、血肌酐(Scr)、血尿素氮(BUN)、尿蛋白定量(24 h)(24 h-UP)]、临床疗效以及治疗前后的免疫指标(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))、用药安全性。结果治疗后,联合组中医症状积分、肾功能指标(CysC、Scr、BUN、24 h-UP)、CD8^(+)均低于对照组,CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)及总有效率均高于对照组(P<0.05);2组不良反应发生率差异无统计学意义(P>0.05)。结论对肺肾气虚型CGN患者采用补中益气、益肾养肺汤联合常规西药治疗能有效减轻其临床症状,改善肾功能及增强免疫力,且疗效显著、安全性高。Objective To explore the clinical effect of Buzhong Yiqi,Yishen Yangfei decoction combined with conventional western medicine in the treatment of chronic glomerulonephritis(CGN)deficiency type of lung and kidney Qi.Methods A total of 102 patients with CGN of lung kidney Qi deficiency type admitted to Wenzhou Traditional Chinese Medicine Hospital from April 2020 to September 2021 were selected and divided into a control group and a combination group,with 51 cases in each group,according to the treatment method.The control group received conventional western medicine treatment,while the combined group received conventional western medicine treatment with combined.The traditional Chinese medicine(TCM)symptom score,renal function indexes[serum cystatin C(CysC),serum creatinine(Scr),blood urea nitrogen(BUN),24-hour urinary protein quantification(24h-UP)]before and after treatment,clinical efficacy,immune indexes(CD3^(+),CD4^(+),CD8^(+),and CD4^(+)/CD8^(+))before and after treatment and drug safety were compared between the two groups.Results After treatment,TCM symptom score,renal function index(CysC,Scr,BUN,24h-UP)and CD8^(+)in combination group were lower than those in control group,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and total effective rate were higher than those in control group(P<0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion For patients with CGN of lung kidney Qi Deficiency type,Buzhong Yiqi,Yishen Yangfei decoction with conventional western medicine can effectively reduce their clinical symptoms,improve renal function and enhance immunity,and the curative effect is remarkable and the safety is high.
