摘 要:目的:探究用免疫抑制法、化学发光法和胶乳免疫比浊法三种方法测定测肌酸激酶同工酶(CK-MB)的结果差异以及免疫抑制法定结果在不同疾病中假性升高的原因。方法:选取2021年于本院就诊的CK-MB免疫抑制法测定结果大于危急值(100U/L)30例患者血清,做化学发光法测定和胶乳免疫比浊法比较结果差异。分析该30例病例是否CK-MB活性为假性升高,并分析该30例病例诊断结果CK-MB活性升高的原因。结果:该30例免疫抑制法测得CK-MB活性均是危急值的病例用化学发光法检测结果升高的只有5例,胶乳免疫比浊法升高的只有4例。该30例中有4例用三种方法检测结果均升高,大部分病例为CK-MB活性假性升高,肿瘤等疾病因产生巨CK或CK-BB干扰免疫抑制法而导致检测结果升高。结论:许多疾病会导致CK-MB活性假性升高,化学发光法与胶乳免疫比浊法可以减少干扰。化学发光法和胶乳比浊法结果检出率做卡方检验,P>0.05,差异未见统计学意义,而胶乳比浊法更便宜、方便,更适合用于临床筛选试验。Objective:To investigate the differences in the rsults of creatine kinase isoenzymes(ck-mb)by immunosuppression,chemiluminescence and latex immunoturbidimetry,and the resons for the pseudo-elevation of immunosuppressive results in different diseases.Methods:The serum of 30patients whose mmunosuppressive assay results were greater than the critical value were selected in 2021,and the differences in results were compared by chemiluminescence and latex immunoturbidimetry.Analyze whether the 30 cases have false elevated CK-MB.The diagnostic results of the 30 cases and the reasons for the increase of CK-MB activity were analyzed.Results:Of these 30 immunosuppressive cases,the CKMB activity was all critical in only 5 cases and the tesults of latex immunoturbidimetry were elevated by chemiluminescence detection,and only 5 cases were increased by latex immunoturbidimetry.In 4 of these 30 cases,the test results were elevated by three methods,most of which were falsely elevated in CK-MB activity,and tumors andother diseases would produce huge CK or CK-BB interfered with immunosuppressive methods,resulting in elevated test results.Conclusion:Many diseases can lead to an increase in CK-MB activity,caused by interference with their measurement methods,which can reduce misdiagnosis by quality or immunoelectric methods.
