摘 要:目的探讨血清可溶性凝集素样氧化型低密度脂蛋白受体-1(soluble lectin-like oxidized LDL receptor-1,sLOX-1)、网膜素-1(omentin-1)、基质金属蛋白酶-9(matrix Metalloproteinase-9,MMP-9)水平检测在H型高血压合并急性缺血性脑卒中(acute ischemic stroke,AIS)患者病情及预后评估中的应用价值。方法回顾性选取滁州市第一人民医院60例H型高血压合并AIS患者(研究组)和60例无AIS的H型高血压患者(对照组)作为研究对象。所有患者均进行血清sLOX-1、Omentin-1和MMP-9水平检测,对比两组的检测结果差异。研究组根据预后情况的不同分为预后良好与预后不良两个亚组,分析患者预后不良的影响因素,并通过绘制受试者工作特征曲线(receiver operating characteristic curve,ROC)曲线分析血清sLOX-1、Omentin-1、MMP-9联用对患者预后的预测价值。结果相比对照组,研究组的血清sLOX-1、MMP-9水平更高,Omentin-1水平更低(P<0.05)。相比预后良好组,预后不良组的糖尿病、高脂血症者占比、Hcy、sLOX-1、MMP-9水平及入院NIHSS评分更高,Omentin-1水平更低(P<0.05)。糖尿病、高脂血症、sLOX-1、MMP-9和入院NIHSS评分高均是患者预后不良的危险因素,Omentin-1为保护因素(均P<0.05),Hcy不是患者预后不良的影响因素(P>0.05)。ROC显示血清sLOX-1、Omentin-1、MMP-9诊断患者预后不良的AUC值分别为0.686、0.831和0.832,3项指标联合诊断的AUC值为0.949。与血清sLOX-1、Omentin-1、MMP-9单用相比,3项联合诊断患者预后不良的AUC值更高(Z=2.946、2.564、2.606,均P<0.05)。结论血清sLOX-1、MMP-9水平升高和Omentin-1水平下降与H型高血压合并AIS的发生及病情进展有密切关联,3项指标联用对患者预后不良有较好的预测效能。Objective To explore the application value of serum levels of soluble lectin like oxidized low-density lipoprotein receptor-1(sLOX-1),Omentin-1,and matrix metalloproteinase-9(MMP-9)in evaluating the condition and prognosis of patients with H-type hypertension complicated with acute ischemic stroke(AIS).Methods A retrospective study was conducted on 60 patients with H-type hypertension complicated with AIS(research group)and 60 patients with H-type hypertension without AIS(control group).All patients underwent serum sLOX-1,Omentin-1 and MMP-9 levels testing,and the difference in test results between the two groups was compared.The research group was divided into two subgroups based on different prognosis:good prognosis and poor prognosis.The influencing factors of poor prognosis in patients were analyzed,and the predictive value of serum sLOX-1,omentin-1,MMP-9 combination on patient prognosis were analyzed by ROC curve.Results Compared with the control group,the research group had higher serum levels of sLOX-1 and MMP-9,and lower levels of Omentin-1(P<0.05).Compared with the good prognosis group,the poor prognosis group had higher rates of diabetes,hyperlipidemia,Hcy,sLOX-1,MMP-9 levels and NIHSS scores,and lower levels of Omentin-1(P<0.05).The diabetes,hyperlipidemia,sLOX-1,MMP-9 and NIHSS score were risk factors for poor prognosis,and Omentin-1 was a protective factor(all P<0.05),and Hcy was not a contributing factor to poor patient prognosis(P>0.05).The ROC curve showed that the AUC values of serum sLOX-1,Omentin-1,and MMP-9 in patients with poor prognosis were 0.686,0.831 and 0.832,respectively.The AUC value of the combined diagnosis of the three indicators was 0.949.Compared with the single use of serum sLOX-1,Omentin-1,and MMP-9,the AUC values of patients with poor prognosis were higher in the three combined diagnoses(Z=2.946,2.564,2.606,all P<0.05).Conclusion Elevated serum levels of sLOX-1,MMP-9 and decreased levels of Omentin-1 are closely related to the occurrence and progression of AIS in patients with
