慢性阻塞性肺疾病合并阻塞性睡眠呼吸暂停低通气综合征患者采取双水平正压通气治疗的临床效果

摘　　要：目的:评价双水平正压通气治疗方案,用于治疗慢性阻塞性肺疾病(COPD)合并阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的治疗价值。方法:选取2019年3月至2024年3月厦门医学院附属第二医院收治的COPD合并OSAHS患者80例作为研究对象,按照随机数字表法分为对照组和观察组,每组40例。以持续正压通气治疗为对照,评价双水平正压通气治疗的实施价值。结果:观察组临床疗效高于对照组,差异有统计学意义(P<0.05);治疗后观察组患者血气指标改善幅度较大,肺功能各指标数值较高,且患者夜间呼吸暂停或低通气次数较少,夜间呼吸暂停最长时间较对照组低,差异有统计学意义(P<0.05);治疗后和对照组比较,观察组患者睡眠质量明显更高,差异有统计学意义(P<0.05)。结论:双水平正压通气治疗COPD合并OSAHS患者有显著效果,可提高治疗效果和睡眠质量,改善患者血气指标,提高肺功能而降低夜间呼吸暂停次数。Objective:Evaluate the value of a dual level positive pressure ventilation treatment for patients with chronic obstructive pulmonary disease(COPD)complicated with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods:The study was conducted from March 2019 to March 2024;Eighty patients with COPD and OSAHS were included in the Second Affiliated Hospital of Xiamen Medical College and were grouped according to the“computerized random grouping method”.The implementation value of dual level positive pressure ventilation therapy continuous positive pressure ventilation therapy were compared.Results:The clinical efficacy of the observation group was higher than that of the control group(P<0.05).After treatment,the observation group showed a significant improvement in blood gas indicators,with higher values of lung function indicators.Additionally,patients had fewer nocturnal breathing pauses or low ventilation,and the longest duration of nocturnal breathing pauses was lower than the control group(P<0.05).Compared with the control group,the observation group had significantly higher sleep quality after treatment(P<0.05).Conclusion:Dual level positive pressure ventilation treatment has significant effects on COPD patients with OSAHS,improves treatment efficacy,sleep quality and blood gas indicators,enhances lung function,and reduces the frequency of nighttime respiratory pauses.
