对比负压吸引鞘与普通软镜鞘在输尿管软镜碎石术中的效果

摘　　要：目的:观察对比输尿管软镜碎石手术治疗过程中负压吸引鞘、普通软镜鞘的应用效果。方法:选择2022年5月~2023年5月在本院进行输尿管软镜碎石手术治疗的106例患者作为临床对象,按患者术中所用方法纳入常规组(n=53)、研究组(n=53),其中常规组患者术中使用普通软镜鞘,研究组患者术中使用负压吸引鞘。对比两组置鞘成功率、结石清除效果、围术期指标(术中操作耗时、围术期失血量、住院时间)、肾盂内压、术后即刻感染水平[白细胞计数(WBC)、C反应蛋白(CRP)、降钙素原(PCT)]、术后视觉模拟评分法(VAS)评分、并发症发生率。结果:研究组置鞘成功率、结石清除率与常规组无差异(P>0.05)。研究组术中操作耗时、围术期失血量、住院时间均少于常规组(P<0.05)。研究组肾盂内压以及术后即刻WBC、CRP、PCT表达水平均明显低于常规组(P<0.05)。研究组术后2h、6h、12h、24h的VAS评分低于常规组(P<0.05)。研究组术后体温升高、出血、输尿管损伤、肾绞痛、感染性休克等并发症发生率与常规组无差异(P>0.05)。结论:输尿管软镜碎石手术治疗过程中负压吸引鞘、普通软镜鞘的应用均能够高效、安全的清除结石,但负压吸引鞘能够降低肾盂内压,减少术中出血,加快术中操作,缓解疼痛,减轻围术期感染程度。Objective:To observe and compare the application effect of negative pressure suction sheath and ordinary flexible endoscope sheath in the treatment of flexible ureteroscopic lithotripsy.Methods:106 patients who underwent flexible ureteroscopic lithotripsy in our hospital from May 2022 to May 2023 were selected as the clinical objects.According to the methods used during the operation,they were included in the conventional group(n=53)and the study group(n=53).The patients in the conventional group used ordinary flexible ureteroscopic sheath during the operation,and the patients in the study group used negative pressure suction sheath during the operation.The success rate of sheath placement,stone removal effect,perioperative indicators[intraoperative operation time,perioperative blood loss,hospital stay],intrapelvic pressure,postoperative immediate infection level[white blood cell count(WBC),C-reactive protein(CRP),procalcitonin(PCT)],postoperative visual analogue scale(VAS)score,and complication rate were compared between the two groups.Results:There was no difference in the success rate of sheath placement and stone clearance rate between the study group and the conventional group(P>0.05).The intraoperative operation time,perioperative blood loss and hospital stay of the study group were less than those of the conventional group(P<0.05).The intrarenal pressure and the expression levels of WBC,CRP and PCT immediately after operation in the study group were significantly lower than those in the conventional group(P<0.05).The VAS scores of the study group at 2h,6h,12h and 24h after operation were lower than those of the conventional group(P<0.05).There was no difference in the incidence of postoperative complications such as increased body temperature,bleeding,ureteral injury,renal colic,septic shock between the study group and the conventional group(P>0.05).Conclusion:The application of negative pressure suction sheath and ordinary soft endoscope sheath in the treatment of flexible ureteroscopic lithotripsy can ef
