摘  要:目的:探讨计算机断层扫描(CT)联合磁共振成像(MRI)检查在腰椎间盘突出症(LDH)诊断中的应用效果。方法:选取2023年5月~2024年4月本院收治的疑似LDH患者198例,均接受CT与MRI检查。比较CT与MRI对于LDH的诊断征象,以及单纯MRI、CT与联合检查的诊断效能。结果:与CT检查结果相比,MRI对于脊髓变性、神经根受压水肿、椎间盘变形征象的检出率更高(P<0.05),钙化、椎间盘积气征象的检出率更低(P<0.05)。198例疑似患者确诊为LDH共178例,单纯MRI诊断LDH的敏感度、准确度高于单纯CT(P<0.05)。CT联合MRI诊断LDH的敏感度、特异度、准确度高于单纯CT、单纯MRI(P<0.05)。结论:CT与MRI在LDH征象的评估方面各具优势,两种技术联合应用可以提高LDH的诊断效能。Objective:Exploring the application effect of CT combined with MRI in the diagnosis of lumbar disc herniation(LDH).Methods:198 suspected LDH patients admitted to our hospital from May 2023 to April 2024 were selected,all of whom underwent CT and MRI examinations.Compared the diagnostic features of CT and MRI for LDH,as well as the diagnostic efficacy of MRI alone,CT alone,and combined examination.Results:Compared with CT examination results,MRI has a higher detection rate for signs of spinal cord degeneration,nerve root compression edema,and intervertebral disc deformation(P<0.05),while the detection rate for signs of calcification and intervertebral disc gas accumulation was lower(P<0.05).178 out of 198 suspected patients were diagnosed with LDH.The sensitivity and accuracy of diagnosing LDH with MRI alone were higher than CT alone(P<0.05).The sensitivity,specificity,and accuracy of joint inspection in diagnosing LDH were higher than CT alone and MRI alone(P<0.05).Conclusions:CT and MRI each have their own advantages in evaluating LDH signs,and the combined application of the two techniques can improve the diagnostic efficiency of LDH.
