心肺运动试验结合动态心电图诊断冠心病的临床价值

摘　　要：目的对心肺运动试验结合动态心电图诊断冠心病的临床价值进行探究。方法选择130例疑似冠心病患者作为观察对象,分别接受心肺运动试验、动态心电图检查、心肺运动试验结合动态心电图检查及冠状动脉造影。以冠状动脉造影为金标准,对比动态心电图检查、心肺运动试验、心肺运动试验结合动态心电图检查的诊断效能。结果130例疑似冠心病患者接受冠状动脉造影诊断后,确诊95例阳性患者、35例阴性患者。动态心电图检查诊断80例真阳性、24例真阴性,心肺运动试验诊断82例真阳性、25例真阴性,心肺运动试验结合动态心电图检查诊断90例真阳性、32例真阴性。心肺运动试验结合动态心电图检查的灵敏度为94.74%,特异度为91.43%;心肺运动试验的灵敏度为86.32%,特异度为71.43%;动态心电图检查的灵敏度为84.21%,特异度为68.57%。心肺运动试验结合动态心电图检查的灵敏度、特异度均高于心肺运动试验、动态心电图检查单一检测,差异具有统计学意义(P<0.05);心肺运动试验、动态心电图检查单一检测灵敏度、特异度比较,差异无统计学意义(P>0.05)。结论对疑似冠心病患者应用心肺运动试验结合动态心电图联合检测的诊断效果较好,能够提升疾病诊断灵敏度,为患者治疗提供依据,最终能够为疾病诊断提供价值。Objective To explore the clinical value of cardiopulmonary exercise test combined with Holter monitor in the diagnosis of coronary heart disease.Methods A total of 130 patients with suspected coronary heart disease were selected for observation.They were subjected to cardiopulmonary exercise test,Holter monitor test,and coronary angiography.The results of coronary angiography was used as the gold standard to compare the diagnostic efficacy of dynamic electrocardiography and Holter monitor alone and in combination.Results Among 130 patients with suspected coronary heart disease,coronary angiography diagnosed 95 positive cases and 35 negative cases;Holter monitor diagnosed 80 true positive cases and 24 true negative cases;cardiopulmonary exercise test diagnosed 82 true positive cases and 125 true negative cases;cardiopulmonary exercise test combined with Holter monitor diagnosed 90 true positive cases and 32 true negative cases.The sensitivity and specificity of cardiopulmonary exercise test combined with Holter monitor were 94.74%and 91.43%;the sensitivity and specificity of cardiopulmonary exercise test were 86.32%and 71.43%;the sensitivity and specificity of Holter monitor were 84.21%and 68.57%.The sensitivity and specificity of cardiopulmonary exercise test combined with Holter monitor were higher than that of cardiopulmonary exercise test or Holter electrocardiogram alone,and the difference was statistically significant(P<0.05).There was no significant difference in the comparison of sensitivity and specificity between cardiopulmonary exercise test and Holter monitor(P>0.05).Conclusion The combined of cardiopulmonary exercise test and Holter monitor for patients with suspected coronary heart disease has a good effect,which can improve the sensitivity of disease diagnosis,provide a basis for patient treatment and ultimately provide diagnostic value for disease diagnosis.
