摘  要:目的探索吸入一氧化氮(iNO)在急性细支气管炎婴儿临床治疗中的疗效及其潜在的抗炎和肺功能改善作用。方法收集2016年1月至2023年12月湖北省黄石市妇幼保健院收治的急性细支气管炎患儿118例,按随机号分为对照组和iNO组,各59例。对照组按常规治疗,观察组在常规治疗的基础上增加iNO治疗。测定入院时以及治疗7 d后的肺功能指标及测定静脉血中的白细胞介素6(IL-6)、嗜酸细胞趋化因子11(CCL11)、半胱氨酰白三烯(CysLTs)表达,分析iNO在婴儿急性细支气管炎治疗中的效果。结果iNO组的肺功能改善优于对照组,IL-6、CCL11、CysLTs等炎性因子的表达水平低于对照组,差异有统计学意义(P<0.05)。结论iNO通过改变T细胞的免疫平衡及降低嗜酸性粒细胞的动员和迁移,抑制其向肺的积聚,从而降低肺组织的炎症反应,有利于患者的肺部炎症消退,体现了肺功能改善的优势。Objective To assess the clinical efficacy of inhaled nitric oxide(iNO)in the treatment of acute bronchiolitis in infants,focusing on its potential anti-inflammatory and lung function improvement effects.Methods A total of 118 infants with acute bronchiolitis admitted to Huangshi Maternal and Child Health Hospital from January 2016 to December 2023 were included.Patients were randomly divided into a control group and an observation group,with 59 infants in each group.The control group received conventional treatment,and the observation group received iNO combined with conventional treatment.Pulmonary function indices and the expression levels of interleukin-6(IL-6),C-C motif chemokine ligand 11(CCL11),and cysteinyl leukotrienes(CysLTs)in venous blood were measured at admission and after 7 days of treatment.The effects of iNO in the treatment of acute bronchiolitis were analyzed.Results The improvement in lung function in the iNO group was significantly greater than in the control group.The expression levels of inflammatory factors,including IL-6,CCL11,and CysLTs,were significantly lower in the iNO group compared to the control group(P<0.05).Conclusion iNO showed a positive effect in the clinical treatment of acute bronchiolitis.It modulates the immune balance of T cells and inhibits the accumulation of eosinophils in the lungs by reducing their mobilization and migration.This reduces the inflammatory response in lung tissue facilitating the resolution of lung inflammation and reflecting its advantages in lung function improvement.
