摘　　要：目的探讨频谱多普勒超声评估胎儿肺静脉对胎儿完全性肺静脉异位引流(TAPVC)的诊断价值。方法回顾性分析22例引产后尸检及4例分娩后超声心动图确诊为TAPVC的常规超声与频谱多普勒超声表现,比较常规超声与频谱多普勒超声对TAPVC的诊断效能。结果26例经尸检及产后超声心动图确诊为TAPVC中,心内型7例,心下型5例,心上型14例。不同TAPVC胎儿的心室大小、垂直静脉梗阻以及合并畸形情况之间的差异存在统计学意义(P<0.05)。频谱多普勒超声通过对异常引流血液的流速和方向的精准测量,其对TAPVC的诊断敏感度高于常规超声诊断(P<0.005),通过受试者工作特征(ROC)曲线分析,频谱多普勒超声诊断的曲线下面积(AUC)高于常规超声诊断(P<0.05)。结论TAPVC采取频谱多普勒超声评估,其诊断效能高于常规超声诊断,建议临床推广。Objective To evaluate the diagnostic value of spectral Doppler ultrasound in evaluating fetal pulmonary veins for total anomalous pulmonary venous connection(TAPVC)in fetuses.Methods The findings of conventional ultrasound and spectral Doppler ultrasound in 22 post-induction postmortem cases and 4 post-delivery echocardiography-confirmed cases were retrospectively analyzed,and their diagnostic performance for conventional ultrasound and spectral Doppler ultrasound was compared.Results Among the 26 confirmed TAPVC cases,7 were intracardiac type,5 were subcardiac type,and 14 were supracardiac type,as verified by postmortem and postpartum echocardiography.Statistically significant differences in ventricular size,vertical venous obstruction,and associated malformations were observed among TAPVC fetuses(P<0.005).By accurately measuring the flow velocity and direction of abnormal drainage blood,spectral Doppler ultrasound showed a higher diagnostic sensitivity for TAPVC than conventional ultrasound(P<0.05).By ROC curve analysis,the area under the curve(AUC)for spectral Doppler ultrasound was higher than conventional ultrasound(P<0.05).Conclusion Spectral Doppler ultrasound offers superior diagnostic efficiency for TAPVC compared to conventional ultrasound,and it is recommended for promotion in clinical practice.
