摘　　要：目的探讨关节镜下治疗儿童膝关节内腱鞘巨细胞瘤(GCTTS)的临床疗效。方法回顾性分析2008年7月至2023年2月泉州市正骨医院收治的6例膝关节内GCTTS患儿(弥漫型2例、局限型4例)的临床资料,所有病例均采用关节镜下手术,观察随访期间患儿复发情况,采用Lysholm膝关节评分评估术前和末次随访时膝关节功能。结果随访9个月~11年,平均随访时间(46.2±44.9)个月。末次随访Lysholm膝关节评分为(94.5±3.6)分,优于术前的(40.0±6.3)分(P<0.05)。1例弥漫型患儿于术后9年复发,再次行关节镜下清理手术;其余病例至末次随访未见复发。结论关节镜下彻底切除儿童膝关节内GCTTS具有直观微创、膝关节功能恢复良好等特点;弥漫型GCTTS有一定的复发率,需重视对此类患儿的长期随访。Objective To investigate the clinical efficacy of arthroscopic treatment for giant cell tumor of tendon sheath(GCTTS)in knee joint of children.Methods The clinical data of 6 children with GCTTS of knee joint(2 cases of diffuse type and 4 cases of localized type)admitted to Quanzhou Orthopaedic Traumatological Hospital from July 2008 to February 2023 were retrospectively analyzed.All patients underwent arthroscopic surgery.The recurrence of children during the follow-up period was observed.Lysholm knee score was used to evaluate the knee function before operation and at the last follow-up.Results The patients were followed up for 9 months to 11 years,with an average of(46.2±44.9)months.The Lysholm knee score at the last follow-up was(94.5±3.6)points,which was better than that before operation,(40.0±6.3)points(P<0.05).One patient with diffuse type recurred 9 years after operation and underwent arthroscopic debridement again.No recurrence was found in the remaining cases until the last follow-up.Conclusions Arthroscopic complete resection of GCTTS in children's knee joint has the characteristics of visualized,minimally invasive procedures and good recovery of knee joint function.Diffuse GCTTS has a certain recurrence rate,which should be paid attention to long-term follow-up.
