摘　　要：目的总结卵巢T_(1)WI高信号病变的磁共振特征,提高对其诊断准确性。方法分析72例卵巢T_(1)WI呈高信号(脂肪除外)病变特征。结果72例患者共83个病灶,其中子宫内膜异位囊肿(EC)50个;囊腺瘤(CA)20个(浆液性11个,黏液性9个);出血囊肿(HC)13个。EC中18个出现“花瓣征”,6个出现“卫星囊”;CA中10个出现“渔网征”。采用Lgistic回归分析,“花瓣征”对EC有较高预测价值,比值比(OR)为18,灵敏度21.69%,特异度100%,准确率61.45%。“渔网征”对CA有较高预测价值,OR为62,灵敏度12.05%,特异度100%,准确率87.95%。结论EC可出现“花瓣征”和“卫星囊”,CA可出现“渔网征”。单房椭圆形病变鉴别较困难,应仔细观察囊内、外壁并结合临床信息综合分析。Objective To summarize the magnetic resonance characteristics of ovarian T_(1)WI high-signal lesions and improve the diagnostic accuracy of them.Methods The characteristics of 72 cases of ovarian lesions with high T_(1)WI signal(except fat)were analyzed.Results There were 83 lesions in 72 patients,including 50 cases of endometrial cyst(EC),20 cases of cystadenoma(CA)(11 cases of serous,9 cases of mucous),and 13 cases of hemorrhagic cyst(HC).In EC,18 cases showed“Petal sign”and 6 cases showed“Satellite sac”.10 cases of CA showed“Fishing net sign”.Logistic regression analysis showed that the“Petal sign”had high predictive value for EC,the odds ratio was 18,the sensitivity,specificity,and accuracy were 21.69%,100%,and 61.45%respectively.The“Fishing net sign”has high predictive value for CA,the odds ratio was 62,the sensitivity,specificity,and accuracy were 12.05%,100%,and 87.95%respectively.Conclusion Endometriosis cyst can show“Petal sign”and“Satellite sac”,and cystadenoma can show“Fishing net sign”.It is difficult to distinguish the single locular oval lesion,so the internal and external walls of the capsule should be carefully observed and combined with the clinical information of the patient.
