围产期妇女不同孕周心理健康现状调查、相关因素分析及对妊娠结局的影响

摘　　要：目的 调查围产期妇女不同阶段的心理健康状况、分析相关影响因素及对妊娠结局的影响,为临床开展心理干预提供参考。方法 随机选取2021年1月至2023年6月在本院产检的107例孕早期妇女作为观察对象,分别在孕早期、孕中期、孕晚期、产褥期通过焦虑自评量表(SAS)、抑郁自评量表(SDS)、爱丁堡产后抑郁量表(EPDS)做心理健康状况测评,并调查其影响因素,多因素logistic回归分析相关性。结果 107例调查的孕妇中,34例存在围产期不良心理状况,占比31.48%;心理状况异常组的孕妇的SAS、SDS、EPDS评分均高于心理状况正常组(P<0.05);多因素分析发现,初次妊娠(OR=4.638)、妊娠期合并贫血(OR=6.489)、胎膜早破(OR=2.793)是围产期孕妇不良心理健康状况的影响因素(P<0.05)。结论 孕妇围产期不同阶段心理状况不同,且受妊娠次数、妊娠合并症等多种因素影响,易出现焦虑、抑郁等心理问题,需及时并给予合理的心理疏导和社会支持,改善妊娠结局。Objective To study the mental status of pregnant women in different perinatal stages and the influencing factors of negative emotion,to provide reference for psychological interven-tion.Methods A total of 107 pregnant women accepting physical examination in Jinan Maternal and Child Health Hospital affiliated to Shandong First Medical University from January 2021 to June 2023 wereandom selected The perinatal mental health status of pregnant women was as-sessed by Self-rating Depression Scale(SDS),Self-rating Anxiety Scale(SAS),and Edinburgh Postpartum Depression Scale(EPDS)in the first,second and third trimesters and puerperal peri-od,and the negative emotion and influencing factors were analyzed.Results Among 107 preg-nant women,34(31.48%)had adverse mental state during perinatal period.The scores of SAS,SDS and EPDS in the abnormal group were higher than those in the normal group(P<0.05),and the difference wasstatistically significant.Multivariate analysis showed that the first preg-nancy(OR=4.638),anemia during pregnancy(OR=6.489),and premature rupture of membranes(OR=2.793)were the influential factors for the adverse mental health status of perinatal pregnant women(P<0.05).Conclusion Pregnant women have different psychological condi-tions at different stages of the perinatal period,they are susceptible to anxiety,depression and other psychological problems during perinatal period due to many factors such as the number of pregnancies and complications of pregnancies.Timely and reasonable psychological counseling and social support are needed to improve the quality of pregnancies.
