自拟滋阴泻火汤加减治疗快进展型性早熟女童的效果及对性激素水平的影响

摘　　要：目的研究自拟滋阴泻火汤加减治疗快进展型性早熟(PP)女童的效果及对性激素水平的影响。方法选取2021年1月至2024年1月在郑州大学第一附属医院接受治疗的81例快进展型PP女童作为研究对象,按随机数表法分为对照组40例和研究组41例。对照组患儿采用注射用醋酸亮丙瑞林缓释球治疗,研究组患儿在对照组治疗的基础上采用自拟滋阴泻火汤治疗,两组患儿均治疗6个月。比较两组患儿治疗6个月后的临床疗效,以及治疗前后的中医证候积分(主症总积分、次症总积分)、性激素(雌二醇、促卵泡激素、黄体生成素)水平、卵巢、子宫容积和骨龄指数,同时比较两组患儿治疗期间的不良反应发生情况。结果治疗6个月后,研究组患儿的总有效率为87.80%,明显高于对照组的67.50%,差异有统计学意义(P<0.05);治疗前,两组患儿的中医证候积分和性激素水平比较差异均无统计学意义(P>0.05);治疗6个月后,两组患儿的中医证候积分、性激素水平均降低,且研究组患儿的主症总积分、次症总积分、雌二醇、促卵泡激素、黄体生成素分别为(2.71±0.32)分、(1.86±0.20)分、(13.49±2.18)pg/mL、(5.03±0.60)mIU/mL、(5.13±0.62)mIU/mL,明显低于对照组的(3.48±0.43)分、(2.25±0.29)分、(18.54±2.93)pg/mL、(8.19±1.13)mIU/mL、(9.60±1.75)mIU/mL,差异均有统计学意义(P<0.05)。治疗前,两组患儿的卵巢、子宫容积和骨龄指数比较差异均无统计学意义(P>0.05);治疗6个月后,两组患儿的卵巢、子宫容积和骨龄指数均缩小,且研究组患儿的卵巢、子宫容积和骨龄指数分别为(1.42±0.21)mL、(1.33±0.20)mL、0.91±0.14,明显小于对照组的(2.03±0.28)mL、(1.60±0.21)mL、1.03±0.17,差异均有统计学意义(P<0.05);两组患儿治疗期间均无明显不良反应发生。结论自拟滋阴泻火汤加减治疗可提高快进展型PP女童的治疗效果,降低性激素水平,延缓第二性征发育,改善骨龄�Objective To study the effect of self-made Ziyin Xiehuo Decoction in the treatment of girls with rapid progressive precocious puberty(PP)and its influence on the level of sex hormones.Methods Eighty-one girls with rapid progressive PP who received treatment at the First Affiliated Hospital of Zhengzhou University from January 2021 to January 2024 were selected and randomly divided into a control group of 40 cases and a study group of 41 cases using a random number table method.Children in the control group were treated with injection of sustained-release leuprorelin acetate balls,while those in the study group were treated with self-made Ziyin Xiehuo Decoction on the basis of the treatment in the control group.Both groups of children were treated for 6 months.The clinical efficacy of two groups after 6 months of treatment were compared,as well as the TCM syndrome scores(total score of main symptoms and total score of secondary symptoms),sex hormone(estradiol,follicle stimulating hormone,luteinizing hormone)levels,ovarian volume,uterine volume,and bone age index before and after treatment.At the same time,the incidence of adverse reactions during the treatment was compared between the two groups.Results After 6 months of treatment,the total effective rate of the study group was 87.80%,which was significantly higher than 67.50%of the control group(P<0.05).Before treatment,there was no significant difference in TCM syndrome score and sex hormone level between the two groups(P>0.05).After 6 months of treatment,the TCM syndrome scores and sex hormone levels of children in both groups were reduced;the total score of main disease,total score of secondary disease,estradiol,follicle stimulating hormone,and luteinizing hormone were(2.71±0.32)points,(1.86±0.20)points,(13.49±2.18)pg/mL,(5.03±0.60)mIU/mL and(5.13±0.62)mIU/mL,which were significantly lower than(3.48±0.43)points,(2.25±0.29)points,(18.54±2.93)pg/mL,(8.19±1.13)mIU/mL,and(9.60±1.75)mIU/mL in the control group(P<0.05).Before treatment,there was no signifi
