自拟中药熏洗方联合雷火灸在肛周脓肿术后患者中的应用

摘　　要：目的:探讨肛周脓肿术后患者联用自拟中药熏洗方、雷火灸的临床疗效。方法:选取2021年8月~2023年8月期间于本院行肛周脓肿术患者96例作为研究对象。随机将患者分为对照组和观察组,每组各48例。对照组予以雷火灸治疗;观察组予以自拟中药熏洗方联合雷火灸治疗。分析比较两组的临床疗效、临床症状改善情况(创面愈合时间、腐肉完全脱离时间、创面缩小率)、疼痛程度[视觉模拟量表(VAS)]、水肿评分、肉芽组织评分、肛门功能、炎症因子[白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)]。结果:观察组的治疗总有效率高于对照组(P<0.05)。观察组的创面愈合时间、腐肉完全脱离时间短于对照组,创面缩小率高于对照组(P<0.05)。治疗后,观察组的VAS评分、水肿评分、肉芽组织评分低于对照组(P<0.05)。治疗后,观察组的直肠肛管抑制反射阈、肛管最大收缩压、肛管静息压均高于对照组(P<0.05)。治疗后,观察组的血清IL-6、IL-8、TNF-α水平均低于对照组(P<0.05)。结论:肛周脓肿术后联合应用自拟中药熏洗方和雷火灸治疗,能提高临床治疗效果,加速创面愈合,改善肉芽组织生长,缓解水肿,减轻疼痛程度,并可改善肛门功能,降低炎症反应。Objective:To investigate the clinical efficacy of combined Chinese herbal fumigation prescription and thunder fire moxibustion in patients with perianal abscess after operation.Methods:96 patients who underwent perianal abscess surgery in our hospital from August 2021 to August 2023 were selected as the study objects.The patients were randomly divided into control group and observation group,with 48 cases in each group.The control group was treated with lightning moxibustion;The observation group was treated with Chinese herbal fumigation recipe combined with thunder fire moxibustion.The clinical efficacy,improvement of clinical symptoms(wound healing time,total detachment time of rotting flesh,wound reduction rate),pain degree[visual analogue scale(VAS)],edema score,granulation tissue score,anal function,inflammatory factors[interleukin-6(IL-6),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α)and growth factors[epidermal growth factor(EGF),vascular endothelial growth factor(VEGF)]were analyzed and compared between the two groups.Results:The total effective treatment rate in the observation group was higher than that in the control group(P<0.05).The wound healing time and complete detachment time of putrefaction in the observation group were shorter than those in the control group,and the wound reduction rate was higher than that in the control group(P<0.05).After treatment,the VAS score,edema score,and granulation tissue score in the observation group were lower than those in the control group(P<0.05).After treatment,the rectal anal inhibitory reflex threshold,maximum anal contractile pressure,and resting anal pressure in the observation group were higher than those in the control group(P<0.05).After treatment,the serum levels of IL-6,IL-8,and TNF-αin the observation group were lower than those in the control group(P<0.05).Conclusion:The combined application of traditional Chinese medicine fumigation recipe and fire moxibustion after the operation of perianal abscess can improve the clinical therapeutic effe
