分泌性中耳炎患者纯音听阈及宽频声导抗吸收率特点分析

摘　　要：目的:分析分泌性中耳炎(SOM)患者纯音听阈及宽频声导抗(WBT)吸收率特点分析。方法:回顾性分析2022年8月-2024年8月在本院确诊的102例SOM单耳患病患者的临床资料,共102耳,为SOM组。另选同时段在我院体检结果健康的102例志愿者作为对照组。采用临床听力仪进行纯音听阈测试,记录所有研究对象0.5~4.0 kHz下纯音听阈均值(PTA);采用中耳分析系统进行WBT测试,记录所有研究对象鼓室压、鼓室图型、声顺值、0.5~4.0 kHz WBT吸收率。结果:对照组研究对象鼓室图型均为正常型,SOM组患者鼓室图型大多数为B型(86.27%);SOM组患者鼓室压绝对值、各频率PTA均明显大于对照组,声顺值明显小于对照组(P<0.05)。两组外耳道容积比较,差异无统计学意义(P>0.05)。两组研究对象WBT吸收率在0.5~4.0 kHz各个频率下均表现出明显差异,在任一相同频率下,对照组研究对象WBT吸收率均明显大于SOM组患者(P<0.05)。结论:不同频率下PTA和WBT吸收率与SOM发病有一定联系,可通过检测上述指标对SOM进早期诊断。Objective:To analyze characteristics of pure-tone threshold and anti-absorption rate of wide band tympanometry(WBT)in patients with secretory otitis media(SOM).Methods:A retrospective analysis was performed on the clinical data of 102 patients(102 ears,SOM group)with monaural SOM and 102 healthy volunteers(control group)in the hospital between August 2022 and August 2024.The pure tone audiometry was conducted by clinical audiometer to record pure tone average(PTA)under 0.5-4.0 kHz.WBT was conducted by middle ear analysis system to record tympanic pressure,tympanic pattern,acoustic compliance value and WBT absorption rates under 0.5-4.0 kHz.Results:The tympanic patterns in control group were all normal,while most of which in SOM group were type B(86.27%).The absolute value of tympanic pressure and PTA under different frequencies in SOM group were significantly greater than those in control group,acoustic compliance value was significantly lower than that in control group(P<0.05),but there was no significant difference in external auditory canal volume between the two groups(P>0.05).There were significant differences in WBT absorption rates under different frequencies(0.5-4.0 kHz)between the two groups.At any of the same frequency,WBT absorption rate in control group was significantly higher than that in SOM group(P<0.05).Conclusion:There is certain relationship between PTA,WBT absorption rates under different frequencies and SOM onset,and detection of the above indexes can be applied in the early diagnosis of SOM.
