摘　　要：目的:探究抗核抗体(ANA)、抗角蛋白抗体(AKA)联合抗环瓜氨酸肽(CCP)抗体检测在类风湿性关节炎(RA)诊断中的应用效果。方法:回顾性分析南阳市中心医院2021年8月~2023年8月接收的107例疑似RA患者资料,以美国风湿病学会RA分类结果为诊断“金标准”,分析ANA、AKA、抗CCP抗体检测以及三抗体联合检测对RA的诊断效能及其诊断准确性差异。结果:三抗体联合检测诊断RA的敏感度、特异度、RA预测值、非RA预测值、准确度分别为95.83%、85.71%、93.24%、90.91%、92.52%,Kappa值为0.827。三抗体联合检测敏感度、准确度高于ANA检测、AKA检测(P<0.05)。结论:ANA、AKA联合抗CCP抗体检测在RA诊断中具有较高的敏感度、特异度和准确性,应用三抗体联合检测方式有利于临床早期诊断RA。Objective:To explore the application effect of antinuclear antibody(ANA),anti-keratin antibody(AKA)combined with anti-cyclic citrullinated peptide(CCP)antibody detection in the diagnosis of rheumatoid arthritis(RA).Methods:The clinical data of 107 patients with suspected RA in Nanyang Central Hospital were retrospectively analyzed from August 2021 to August 2023.American College of Rheumatology RA classification was used as the“gold standard”for diagnosis,and the differences in diagnostic efficiency and diagnostic accuracy of ANA,AKA,anti-CCP antibody detection and combined detection of three antibodies for RA were analyzed.Results:The sensitivity,specificity,RA predictive value,non-RA predictive value and accuracy of combined detection of three antibodies in the diagnosis of RA were 95.83%,85.71%,93.24%,90.91%and 92.52%respectively,and the Kappa value was 0.827.The sensitivity and accuracy of combined detection of three antibodies were higher than those of ANA detection and AKA detection(P<0.05).Conclusion:The detection of ANA,AKA combined with anti-CCP antibody has high sensitivity,specificity and accuracy in the diagnosis of RA.The combined detection of three antibodies is beneficial to the clinical early diagnosis of RA.
