摘　　要：目的:探讨经鼻高流量湿化氧疗对慢性阻塞性肺疾病(COPD)合并慢性左心衰竭患者(HFNC)的治疗效果。方法:回顾性收集2021年1月至2023年6月期间本院收治的98例COPD合并慢性左心衰竭患者的临床资料。根据治疗方案的不同将患者分为对照组(经鼻导管吸氧治疗,n=49)与观察组(HFNC治疗,n=49)。分析对比两组的呼吸困难指数(mMRC评分)、心功能、血气参数及安全性。结果:治疗后,观察组mMRC评分、NT-proBNP、PaCO_(2)水平显著低于对照组,LVEF、PaO_(2)水平显著高于对照组(P<0.05)。观察组的不良事件总发生率显著低于对照组(P<0.05)。结论:HFNC应用在COPD合并慢性左心衰竭患者的治疗中,能缓解患者的呼吸困难症状,改善心功能指标和血气参数,提高安全性。Objective:To investigate the effect of nasal high flow humidified oxygen therapy on chronic obstructive pulmonary disease(COPD)complicated with chronic left heart failure(HFNC).Methods:The clinical data of 98 patients with COPD combined with chronic left heart failure admitted to our hospital from January 2021 to June 2023 were retrospectively collected.Patients were divided into control group(nasal catheter oxygen therapy,n=49)and observation group(HFNC treatment,n=49)according to different treatment regiments.The dyspnea index(mMRC score),cardiac function,blood gas parameters and safety were analyzed and compared between the two groups.Results:After treatment,the mMRC score,NT-proBNP and PaCO_(2) levels in the observation group were significantly lower than those in the control group,and the LVEF and PaO_(2) levels were significantly higher than those in the control group(P<0.05).The total incidence of adverse events in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:The application of HFNC in the treatment of COPD patients with chronic left heart failure can alleviate dyspnea symptoms,improve cardiac function indexes and blood gas parameters,and improve safety.
