经阴道三维能量多普勒超声联合经腹超声对剖宫产术后瘢痕妊娠分型的诊断

摘　　要：目的:探讨剖宫产术后瘢痕妊娠(Cesarean scar pregnancy,CSP)由经阴道三维能量多普勒超声+经腹超声分型的准确度。方法:回顾性收集2022年1月至2023年8月期间本院收治的78例CSP患者的临床资料。所有患者入院后行经阴道三维能量多普勒超声、经腹超声检查,并以临床手术诊断结果作为分型的金标准,比较经阴道三维能量多普勒超声、经腹超声、二者联合诊断CSP分型一致性及准确度。结果:CSP患者中Ⅰ型22例,Ⅱ型37例,Ⅲ型19例。经腹超声诊断CSP分型结果与临床结果的一致性较强(Kappa=0.696,P<0.05);经阴道三维能量多普勒超声诊断CSP分型结果与临床结果的一致性较强(Kappa=0.759,P<0.05);二者联合诊断CSP分型结果与临床结果的一致性强(Kappa=0.919,P<0.05);二者联合检查诊断CSP分型的准确度均高于单独经腹超声、经阴道三维能量多普勒超声检查,差异均有统计学意义(P<0.05)。结论:较单独经腹超声、经阴道三维能量多普勒超声检查,二者联合诊断CSP分型的一致性更强,准确度更高。Objective:To investigate the accuracy of transvaginal three-dimensional energy Doppler ultrasonography plus transabdominal ultrasonography in classification of scar pregnancy(Cesarean scar pregnancy,CSP)after cesarean section.Methods:The clinical data of 78 patients with CSP admitted to our hospital from January 2022 to August 2023 were retrospectively collected.After admission,all patients underwent transvaginal three-dimensional energy Doppler ultrasound and transabdominal ultrasound.The results of clinical surgery were used as the gold standard for classification.The consistency and accuracy of transvaginal three-dimensional energy Doppler ultrasound,transabdominal ultrasound and their combined diagnosis of CSP were compared.Results:Among the CSP patients,22 cases were type I,37 cases were type II and 19 cases were type III.The results of CSP classification by transabdominal ultrasound were consistent with clinical results(Kappa=0.696,P<0.05).The results of transvaginal three-dimensional energy Doppler ultrasound diagnosis of CSP were consistent with clinical results(Kappa=0.759,P<0.05).The results of the combined diagnosis of CSP were consistent with the clinical results(Kappa=0.919,P<0.05).The accuracy of CSP classification by the combined examination was higher than that by abdominal ultrasound and transvaginal three-dimensional energy Doppler ultrasound alone,and the differences were statistically significant(P<0.05).Conclusion:Compared to standalone transabdominal ultrasound and transvaginal three-dimensional energy Doppler ultrasound,the combination examination has stronger consistency and higher accuracy in diagnosing CSP classification.
