鼓室成形术联合开放式乳突根治术治疗慢性中耳炎的效果及对患者听力变化的影响

摘　　要：目的探讨鼓室成形术联合开放式乳突根治术治疗慢性中耳炎的效果及对患者听力变化的影响。方法选取2021年1月-2024年7月丰城市中医院收治的慢性中耳炎患者52例作为研究对象,按照随机数字表法将其分为对照组和研究组,每组26例。对照组行经典乳突根治术治疗,研究组行鼓室成形术联合开放式乳突根治术治疗。比较两组治疗效果、听力恢复情况(气导听力阈值、骨导听力阈值、气骨导听差值)、临床症状改善时间(鼓膜充血症状消失时间、穿孔愈合时间、耳痛症状消失时间、中耳积脓消失时间)、不良反应发生率(鼓室神经损伤、中外耳道狭窄、感染、鼓膜穿孔)。结果研究组治疗总有效率为96.15%,高于对照组的73.08%(P<0.05);研究组术后气导听力阈值、骨导听力阈值、气骨导听差值均低于对照组(P<0.05);研究组鼓膜充血症状消失时间、穿孔愈合时间、耳痛症状消失时间、中耳积脓消失时间短于对照组(P<0.05);研究组不良反应发生率低于对照组(P<0.05)。结论鼓室成形术联合开放式乳突根治术治疗慢性中耳炎的临床效果确切,能够改善患者听力情况,加快患者临床症状改善速度,且安全性较高。Objective To investigate the effect of tympanoplasty combined with open radical mastoidectomy in the treatment of chronic otitis media and its effect on hearing changes.Methods A total of 52 patients with chronic otitis media admitted to Jiangxi Fengcheng Hospital of Traditional Chinese Medicine from January 2021 to July 2024 were selected as the research objects.According to the random number table method,they were divided into control group and study group,with 26 patients in each group.The control group was treated with classical radical mastoidectomy,and the study group was treated with tympanoplasty combined with open radical mastoidectomy.The therapeutic effect,hearing recovery(air conduction hearing threshold,bone conduction hearing threshold,air bone conduction hearing difference),clinical symptom improvement time(tympanic membrane congestion symptom disappearance time,perforation healing time,ear pain symptom disappearance time,middle ear empyema disappearance time),and incidence of adverse reactions(tympanic nerve injury,middle and external auditory canal stenosis,infection,tympanic membrane perforation)were compared between the two groups.Results The total effective rate of treatment in the study group was 96.15%,which was higher than 73.08%in the control group(P<0.05).The postoperative air conduction hearing threshold,bone conduction hearing threshold and air-bone conduction hearing difference in the study group were lower than those in the control group(P<0.05).The disappearance time of tympanic membrane congestion symptoms,perforation healing time,ear pain symptoms and middle ear empyema in the study group were shorter than those in the control group(P<0.05).The incidence of adverse reactions in the study group was lower than that in the control group(P<0.05).Conclusion Tympanoplasty combined with open radical mastoidectomy has a definite clinical effect in the treatment of chronic otitis media,which can improve the hearing of patients,accelerate the improvement of clinical symptoms,and has high saf
