摘 要:目的研究血液透析(HD)联合血液灌流(HP)治疗对慢性肾病患者血清钙磷、血脂和肾功能指标的影响。方法以2021年7月-2023年10月万年县人民医院肾内科收治的68例慢性肾病患者为研究对象,经随机数字表法分为对照组(34例)和观察组(34例)。对照组行HP治疗,观察组则采用HD联合HP治疗,比较两组血清钙、磷水平、炎性指标[C-反应蛋白(CRP)、白介素(IL-6)]、血脂水平[甘油三酯(TG)、总胆固醇(TC)]、肾功能指标[血清肌酐(Scr)、血清尿素氮(BUN)]。结果与治疗前比较,两组治疗后血清钙含量及Scr、BUN水平更高,磷含量及CRP、IL-6、TG、TC水平更低,且与对照组比较,观察组血清钙含量更高、磷含量及CRP、IL-6、TG、TC、Scr、BUN更低(P<0.05)。结论HD联合HP可纠正慢性肾病患者的钙磷代谢平衡,下调其炎性指标及血脂水平,缓解肾功能损伤。Objective To study the effect of hemodialysis(HD)combined with hemoperfusion(HP)on serum calcium,phosphorus,blood lipid and renal function in patients with chronic kidney disease.Methods Using the random number table method,totally 68 patients with chronic kidney disease admitted to the Department of Nephrology,Wannian County People's Hospital from July 2021 to October 2023 were divided into control group(34 patient)and observation group(34 patient).In the control group,patients were treated with HP,while in the observation group,patient were treated with HD combined with HP.The serum calcium and phosphorus levels,inflammatory indexes[C-reactive protein(CRP),interleukin(IL-6)],blood lipid levels[triglyceride(TG),total cholesterol(TC)],renal function indexes[serum creatinine(Scr),serum urea nitrogen(BUN)]were compared between the two groups.Results Compared with before treatment,the serum calcium content,Scr and BUN levels were higher,and the phosphorus content,CRP,IL-6,TG and TC levels were lower in the two groups after treatment,while compared with the control group,the serum calcium content was higher,and the phosphorus content,CRP,IL-6,TG,TC,Scr and BUN were lower in the observation group(P<0.05).Conclusion HD combined with HP can correct the balance of calcium and phosphorus metabolism in patients with chronic kidney disease,down-regulate their inflammatory indicators and blood lipid levels,and alleviate renal function damage.
