摘　　要：目的探究促甲状腺激素受体抗体(TRAb)、促甲状腺激素(TSH)及干扰素(IFN)-γ、白细胞介素(IL)-17水平变化在甲状腺功能亢进患者诊断中的应用效果。方法选取2018年10月-2022年9月罗定市人民医院收治的疑似甲状腺功能亢进患者506例作为研究组,选取同一时间段内该院健康体检者150例作为对照组。比较两组TRAb、TSH、IFN-γ和IL-17水平,以病理诊断结果作为检测“金标准”,分析不同指标诊断甲状腺功能亢进的灵敏度、特异度、阳性预测值、阴性预测值、准确率、漏诊率、误诊率。结果研究组TRAb、IL-17水平高于对照组,TSH、IFN-γ水平低于对照组(P<0.05)。TRAb、TSH、IFN-γ和IL-17水平联合检测诊断甲状腺功能亢进的灵敏度、特异度、阳性预测值、阴性预测值、准确率均高于上述指标单独检测,且漏诊率、误诊率低于单独检测(P<0.05)。结论TRAb、TSH、IFN-γ和IL-17水平变化在甲状腺功能亢进患者诊断中的应用效果确切,便于评估患者病情状况,为后期治疗提供准确参考依据。Objective To explore the application effect of changes of thyroid stimulating hormone receptor antibody(TRAb),thyroid stimulating hormone(TSH),interferon(IFN)-γand interleukin(IL)-17 levels in the diagnosis of patients with hyperthyroidism.Methods A total of 506 patients with suspected hyperthyroidism admitted to Luoding People's Hospital from October 2018 to September 2022 were selected as the study group,and 150 healthy subjects in the hospital during the same period were selected as the control group.The levels of TRAb,TSH,IFN-γand IL-17 were compared between the two groups.The pathological diagnosis results were used as the gold standard for detection,the sensitivity,specificity,positive predictive value,negative predictive value,accuracy,missed diagnosis rate and misdiagnosis rate of different indicators in the diagnosis of hyperthyroidism were analyzed.Results The levels of TRAb and IL-17 in the study group were higher than those in the control group,and the levels of TSH and IFN-γwere lower than those in the control group(P<0.05).The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of combined detection of TRAb,TSH,IFN-γand IL-17 levels in the diagnosis of hyperthyroidism were higher than those of the above indexes alone,and the missed diagnosis rate and misdiagnosis rate were lower than those of the above indexes alone(P<0.05).Conclusion The changes of TRAb,TSH,IFN-γand IL-17 levels are effective in the diagnosis of patients with hyperthyroidism.It is convenient to evaluate the condition of patients and provide accurate reference for later treatment.
