心电图对急性ST段抬高型心肌梗死罪犯血管定位及合并缺血性心肌病的诊断价值

摘　　要：目的研究心电图对急性ST段抬高型心肌梗死(STEMI)罪犯血管定位及合并缺血性心肌病(ICM)的诊断价值。方法以2021年7月-2023年6月于都县人民医院收治的76例STEMI患者为研究对象,统计心电图对STEMI罪犯血管的定位准确率,以最终诊断结果为金标准,分析心电图检测STEMI罪犯血管的诊断效能,同时计算心电图对STEMI患者合并ICM的诊断效能。结果心电图对急性前壁心肌梗死(AAMI)患者罪犯血管的定位准确率高于急性下壁心肌梗死(AIMI)患者(P<0.05)。心电图检测STEMI罪犯血管的诊断准确性为82.89%,敏感度为82.86%,特异度为83.33%。以心脏彩超结果为金标准,心电图对STEMI患者合并ICM的诊断准确性为86.84%,敏感度为86.54%,特异度为87.50%。结论心电图在STEMI罪犯血管定位及合并ICM检出中均具有良好的诊断效能。Objective To study the diagnostic value of electrocardiogram(ECG)in the localization of culprit vessels and ischemic cardiomyopathy(ICM)in patients with acute STsegment elevation myocardial infarction(STEMI).Methods A total of 76 STEMI patients admitted to Yudu County People's Hospital from July 2021 to June 2023 were enrolled in the study.The accuracy of electrocardiogram in the location of STEMI culprit vessels was counted.The final diagnostic results were used as the gold standard.The diagnostic efficacy of electrocardiogram in detecting STEMI culprit vessels was analyzed,and the diagnostic efficacy of electrocardiogram in STEMI patients with ICM was calculated.Results The accuracy of electrocardiogram in the localization of culprit vessels in patients with acute anterior myocardial infarction(AAMI)was higher than that in patients with acute inferior myocardial infarction(AIMI)(P<0.05).The diagnostic accuracy of electrocardiogram in detecting STEMI culprit vessels was 82.89%,the sensitivity was 82.86%,and the specificity was 83.33%.Taking the results of echocardiography as the gold standard,the diagnostic accuracy of electrocardiogram for STEMI patients with ICM was 86.84%,the sensitivity was 86.54%,and the specificity was 87.50%.Conclusion Electrocardiogram has good diagnostic efficacy in the localization of STEMI culprit vessels and the detection of combined ICM.
