从“精血虚衰”论治血糖波动

摘　　要：血糖波动是糖尿病患者慢性并发症发生发展的主要因素。临床观察发现,血糖波动较多出现于年老久病者或青年早发糖尿病及形体消瘦者,即多为久病消耗或先天不足,因此考虑血糖波动的产生与中医理论中的“虚劳”相关。为探究血糖波动与虚劳的相关性,利用文献检索、查询中文数据库,筛选有关血糖波动的文献,通过对所查阅文献中血糖波动的发病人群、影响因素及常见证型进行整理、分析,总结出糖尿病中虚劳相关证型对血糖波动影响最大。金元名家朱丹溪在《黄帝内经》“阴虚致衰”的理论基础上确立“精血虚衰”的学术思想,首创“补精养形治衰”的虚劳治则。虚劳则必耗其精血,“精血虚衰”理论可指导论治血糖波动,为中医治疗糖尿病患者的血糖波动提供新的视角。Blood glucose fluctuation is a major factor in the occurrence and development of chronic complications in diabetes patients.Clinical observation has found that blood glucose fluctuation mostly occurs in elderly patients with aeipathia or young people with early-onset diabetes and emaciator,that is,most of them are chronically ill or congenital deficiencies.Therefore,it is considered that the generation of blood glucose fluctuation is related to the“consumptive disease”in the theory of traditional Chinese medicine.In order to explore the correlation between blood glucose fluctuation and consumptive disease,literature search and query Chinese database were used to screen the literature on blood glucose fluctuation.Through sorting out and analyzing the incidence groups,influencing factors and common syndrome types of blood glucose fluctuation in the literature consulted,it is concluded that the type of syndrome related to consumptive disease in diabetes has the greatest influence on blood glucose fluctuation.Zhu Danxi,a famous doctor of Jin Yuan,established the academic thought of“Essence and Blood Deficiency”on the basis of the theory of“Yin deficiency leading to decline”in Huangdi Neijing,and pioneered the principle of“tonifying essence and nourishing body for treatment of decline”.Consumptive disease will deplete their essence and blood.The theory of“Essence and Blood Deficiency”can guide the treatment of blood glucose fluctuation and provides a new perspective for the treatment of blood glucose fluctuation in patients with diabetes.
