基于“阴火”探析糖尿病便秘治疗

摘　　要：便秘是糖尿病患者临床常见的并发症之一,严重影响患者生存质量,增加血糖和血压的控制难度,易诱发心肌梗死、脑梗死等急性并发症。李东垣创立的“阴火理论”以脾胃虚损为主要病因病机,这与糖尿病便秘的核心病机一致,故以此为出发点探讨阴火产生的病因,并分析阴火与糖尿病便秘病因病机、病位、临床症状等相关性,从而明确“阴火”是该病发生发展的中心环节,提出糖尿病便秘可从健脾升清阳、滋肾泻阴火两方面并依据病情进展分四期论治,以期为临床治疗糖尿病便秘提供新的诊疗思路。Constipation is one of the common clinical complications of diabetic patients.It not only seriously affects the quality of life of patients,but also increases the difficulty of controlling blood glucose and blood pressure,and is easy to induce acute complications such as myocardial infarction and cerebral infarction.The spleen and stomach deficiency characterizes the theory of Yin fire founded by Li Dongyuan as the main etiology and pathogenesis,consistent with the core pathogenesis of diabetic constipation.To take this as a starting point to explore the etiology of yin fire,and analyzes the correlation between Yin fire and the etiology,pathogenesis,location,and clinical symptoms of diabetic constipation,to clarify that Yin fire is the central link in the occurrence and development of this disease.It is proposed that diabetic constipation can be divided into four stages according to the progression of the disease,to provide new ideas for clinical diagnosis and treatment of diabetic constipation.
