摘 要:目的:研究玉蓝降糖胶囊对糖尿病周围神经病变模型大鼠坐骨神经髓鞘损伤的修复作用及其对早期生长反应因子2(EGR2/KROX20)和髓鞘碱性蛋白(MBP)表达的影响,探讨玉蓝降糖胶囊对周围神经病变大鼠髓鞘损伤的保护机制。方法:将96只SD大鼠随机分为正常组、模型组、玉蓝降糖胶囊低剂量组、玉蓝降糖胶囊中剂量组、玉蓝降糖胶囊高剂量组、二甲双胍+依帕司他组,每组16只。正常组大鼠予普通饲料饲养,其余各组大鼠予链脲佐菌素腹腔注射配合高脂高糖饲料喂养,各给药组小鼠均灌胃相应剂量药物。给药结束后检测大鼠机械痛阈值(MNT)及坐骨神经传导速度(SNCV),HE染色法观察大鼠坐骨神经和背根神经节(DRG)组织的病理改变;采用免疫组化法检测小鼠坐骨神经中KROX20、MBP和白介素-6(IL-6)表达水平。结果:模型组大鼠血清糖化血清蛋白(GSP)水平高于正常组(P<0.01);玉蓝降糖胶囊高剂量组大鼠血清GSP水平低于模型组(P<0.01)。模型组大鼠MNT、SNCV均低于正常组,差异有统计学意义(P<0.01);玉蓝降糖胶囊高剂量组、二甲双胍+依帕司他组大鼠MNT、SNCV均高于模型组(P<0.05或P<0.01)。病理学结果显示,与正常组比较,模型组大鼠坐骨神经中出现较多神经髓鞘结构紊乱、轴突萎缩的现象,DRG神经元结构松散,出现空泡变化、染色减弱和尼氏体减少;与模型组比较,玉蓝降糖胶囊低剂量组、玉蓝降糖胶囊中剂量组、玉蓝降糖胶囊高剂量组、二甲双胍+依帕司他组大鼠神经损伤得到了明显改善,DRG神经元染色增强,尼氏体增加。模型组大鼠坐骨神经病变程度HE评分高于正常组(P<0.05);玉蓝降糖胶囊高剂量组大鼠坐骨神经病变程度HE评分低于模型组(P<0.05)。免疫组化结果显示,模型组大鼠坐骨神经中KROX20和MBP表达水平低于正常组(P<0.01),IL-6蛋白表达水平高于正常组(P<0.05);玉蓝降糖胶囊中剂量组、玉蓝降�Objective:To study the effect of Yulan Jiangtang capsules on the repair of myelin injury of the sciatic nerve in diabetic peripheral neuropathy model rats and its impact on the expression of early growth response factor 2(EGR2/KROX20) and myelin basic protein(MBP),and to explore the mechanism of Yulan Jiangtang capsules on the protection of myelin injury in rats with peripheral neuropathy.Method:Totally 96SD rats were randomly divided into normal group,model group,Yulan Jiangtang capsules low-dose group(low-dose group),Yulan Jiangtang capsules medium-dose group(medium-dose group),Yulan Jiangtang capsules high-dose group(high-dose group),and metformin+epalrestat group,with 16 rats in each group.The rats in normal group were fed with normal feed,while the rats in the remaining groups were given streptozotocin by intraperitoneal injection together with high-fat and high-sugar feed,and the mice in each group were gavaged with the corresponding dose of the drug.The mechanical pain threshold(MNT) and sciatic nerve conduction velocity(SNCV) were measured at the end of drug administration,and the pathological changes of sciatic nerve and dorsal root ganglion(DRG) tissues were observed by HE staining;The expression levels of KROX20,MBP,and interleukin-6(IL-6) in the sciatic nerve of rats were measured by immunohistochemistry.Results:The model group showed higher level of glycated serum protein(GSP) than normal group(P<0.01);The high-dose group showed lower level of GSP than model group(P<0.01).The model group showed lower MNT and SNCV than normal group,and the differences were statistically significant( P <0.01);The high-dose group and metformin+epalrestat group showed higher MNT and SNCV than model group(P<0.05 or P<0.01).Pathological results showed that compared with the normal group,more nerve myelin structure disorders and axonal atrophy were observed in the sciatic nerve of rats in the model group,and that the DRG neurons were loose in structure,with vacuolar changes,weakened staining,and reduced nissl body.Compared
