腹针联合雷火灸对盆腔炎性疾病后遗症患者中医证候、血清C反应蛋白及复发率的影响

摘　　要：目的:探究腹针联合雷火灸对盆腔炎性疾病后遗症(SPID)患者中医证候、血清C反应蛋白(CRP)及复发率的影响。方法:将92例SPID患者分为常规组(43例)、联合组(49例),常规组接受西药常规治疗,联合组接受西药常规治疗与腹针联合雷火灸治疗,治疗周期均为8周。对比两组患者的治疗有效率、治疗前后的中医证候积分、盆底局部体征(盆底包块最大直径、盆腔积液最大深度、盆腔体征积分)、血液流变学(血浆黏度、红细胞沉降率、纤维蛋白原含量)、血清炎症因子[CRP、清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]、不良反应及复发率。结果:联合组患者治疗有效率高于常规组(P<0.05);治疗后,联合组患者中医证候积分低于常规组(P<0.05);联合组患者盆底包块最大直径、盆腔积液最大深度、盆腔体征积分均低于常规组(P<0.05);联合组患者血浆黏度、红细胞沉降率、纤维蛋白原含量均低于常规组(P<0.05);联合组患者血清CRP、TNF-α、IL-6水平均低于常规组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);联合组患者复发率低于常规组(P<0.05)。结论:腹针联合雷火灸治疗SPID患者疗效显著,可有效缓解盆底症状,改善血液循环,降低炎症反应和复发率。Objective:To explore the effects of abdominal acupuncture combined with thunder-fire moxibustion on TCM syndromes,serum C reactive protein(CRP) and recurrence rate in patients with sequelae of pelvic inflammatory disease(SPID).Methods:A total of 92 patients with SPID were divided into routine group(43 cases) and combination group(49 cases).Patients were given routine western medicine in routine group,while abdominal acupuncture combined with thunder-fire moxibustion on basis of routine group,in combination group.All patients were treated for 8 weeks.The response rate of treatment,scores of TCM syndromes,local pelvic floor signs(the maximum diameter of pelvic floor mass,the maximum depth of pelvic effusion,scores of pelvic signs),hemorheology(plasma viscosity,erythrocyte sedimentation rate,fibrinogen level),serum inflammatory factors [CRP,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)],adverse reactions and recurrence rate were compared between the two groups.Results:The combination group showed higher response rate of treatment than routine group(P <0.05).After treatment,combination group showed lower scores of TCM syndromes than routine group(P<0.05),and combination group showed lower maximum diameter of pelvic floor mass,maximum depth of pelvic effusion and scores of pelvic signs than routine group(P<0.05).Combination group showed lower plasma viscosity,erythrocyte sedimentation rate and fibrinogen level than routine group(P<0.05),and combination group showed lower levels of CRP,TNF-α and IL-6 than routine group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups( P >0.05).The combination group showed lower recurrence rate than routine group(P<0.05).Conclusion:Abdominal acupuncture combined with thunder-fire moxibustion can provide significant curative effect on SPID patients,relieve pelvic floor symptoms,improve blood circulation,and reduce inflammatory response and recurrence rate.
