从脾“居中央以灌四傍”论身材偏矮病机与辨治

摘　　要：身材偏矮儿童的身高增长空间受限,部分儿童可发展为矮身材,目前多以饮食、睡眠、运动等生活方式调摄进行干预。祖国医学倡导“治未病”的理论思想,可帮助依从性较差的身材偏矮儿童改善身高发育水平。根据《内经》中“脾为孤脏,居中央以灌四傍”的理论观点,并联系身材偏矮发生的病因及临床表现,中医认为其主要病机为“脾失运化,肾气不充”。辨证施治时以脾气健运、脾阴濡养为本,兼顾肾气充盈,同治肝、心、肺脏,使脾润气健、肾气充足、余脏调和,则周身四末得以灌溉濡养,骨髓充盈,骨骼得养,身高增长,以期为身材偏矮的中医治疗带来新的临床辨治思路。Children with shorter stature have limited potential for height increase,and some can be further accentuated to short stature.Currently,interventions are primarily focused on lifestyle adjustments such as diet,sleep,and exercise.Traditional Chinese medicine(TCM)advocates the concept of"preventive treatment"which can assist shorter stature children with poor compliance in improving their height development.According to the"Plain Questions",which states"the spleen locating in the middle and irrigating the other four viscera"and in conjunction with the etiology and clinical manifestations of short stature,TCM considers the main pathogenesis to be"spleen dysfunction and insufficient kidney Qi."The treatment is based on strengthening the spleen's transportation and nourishment,as well as enriching kidney Qi,while also considering the harmony of the liver,heart,and lung functions.This approach aims to moisten the spleen and strengthen its Qi,as well as enrich kidney Qi and harmonize the other organs,thereby nourishing the whole body,filling the marrow,and nourishing the bones,leading to increased height.This paper aims to provide a new clinical diagnostic and treatment perspective for TCM in the treatment of children with shorter stature.
