柴芩承气汤联合乌司他丁对重症急性胰腺炎患者的影响

摘　　要：目的 分析重症急性胰腺炎患者在接受乌司他丁治疗时,采用柴芩承气汤辅助治疗对其胃肠道功能、免疫功能及炎症反应的影响。方法 选取郑州市第七人民医院2023年1月至2024年6月收治的80例重症急性胰腺炎患者,随机分成两组,对照组(n=40例)仅接受乌司他丁治疗,而研究组(n=40例)则在乌司他丁治疗的前提下联合应用柴芩承气汤治疗,比较两组临床疗效、中医证候积分、胃肠道功能、肠道黏膜屏障功能、免疫功能及炎症反应指标。结果 研究组治疗有效率(95.00%)高于对照组(65.00%),差异具有统计学意义(P <0.05)。研究组肠鸣音恢复、腹胀及腹痛消失时间短于对照组,差异具有统计学意义(P <0.05)。治疗后,研究组中医证候积分、内毒素、D-乳酸、二胺氧化酶水平及C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)水平低于对照组,研究组免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)水平高于对照组,差异具有统计学意义(P <0.05)。结论 重症急性胰腺炎患者在接受乌司他丁治疗时联合应用柴芩承气汤辅助治疗能取得显著疗效,不但能及早恢复患者胃肠道功能,显著改善肠道黏膜屏障功能,还能显著提高患者免疫功能,减轻炎症反应。Objective To analyze the impact of Chaiqin Chengqi Decoction as an adjunct therapy to ulinastatin on gastrointestinal function,immune function,and inflammatory response in patients with severe acute pancreatitis.Methods A total of 80 patients with severe acute pancreatitis treated at Zhengzhou Seventh People’s Hospital from January 2023 to June 2024 were randomly divided into two groups:the control group(n=40)received ulinastatin alone,while the study group(n=40)received ulinastatin combined with Chaiqin Chengqi Decoction.The clinical efficacy,Traditional Chinese Medicine syndrome scores,gastrointestinal function,intestinal mucosal barrier function,immune function,and inflammatory response indicators were compared between the two groups.Results The treatment effective rate in the study group(95.00%)was significantly higher than that in the control group(65.00%),with a statistically significant difference(P<0.05).The recovery time for bowel sounds and the resolution time of abdominal distension and pain in the study group were shorter than those in the control group,with statistically significant differences(P<0.05).After treatment,the study group showed lower Traditional Chinese Medicine syndrome scores,levels of endotoxin,D-lactic acid,diamine oxidase,C-reactive protein(CRP),interleukin-6(IL-6),and tumor necrosis factor-alpha(TNF-α)compared to the control group.Additionally,the levels of immunoglobulin A(IgA),immunoglobulin M(IgM),and immunoglobulin G(IgG)were significantly higher in the study group than those in the control group(P<0.05).Conclusion The combined application of Chaiqin Chengqi Decoction with ulinastatin in patients with severe acute pancreatitis significantly enhances therapeutic efficacy,promotes early recovery of gastrointestinal function,markedly improves intestinal mucosal barrier function,and notably boosts immune function while alleviating inflammatory responses.
