摘 要:目的对特应性皮炎(AD)患儿血清特异性过敏原的特征进行分析,为医院预防该疾病提供参考。方法本研究共纳入2021年4月至2024年3月中国人民解放军联勤保障部队第九八八医院确诊为AD的685例患儿作为研究对象进行回顾性分析,检测所有患儿血清总免疫球蛋白E(TIgE)及血清特异性免疫球蛋白E(sIgE)。分析血清TIgE及sIgE的检测结果,将不同年龄组及不同性别AD患儿过敏原阳性率进行对比。结果685例AD患儿中,经检测血清TIgE结果显示为阳性的患儿有490例,阳性率为71.53%,其中血清TIgE数值在200 kU/L以上的患儿有471例,占比68.76%,血清TIgE数值在100~200 kU/L的患儿有19例,占比2.77%;血清sIgE检测为阳性的患儿有428例,阳性率为62.48%,其中男232例,占比33.87%,女196例,占比28.61%;过敏原为1项、2项、3项、4项及以上阳性的患儿分别有178、131、98、80例。牛奶、鸡蛋白在食物性过敏原中阳性率较高;尘螨、猫毛在吸入性过敏原阳性率较高;各个年龄阶段患儿鸡蛋白、牛奶、虾、牛肉、尘螨、猫毛、树组合等及霉菌组合特异性过敏原阳性率经对比,差异均有统计学意义(P<0.05),其中鸡蛋白、牛奶的特异性过敏原阳性率随着年龄的增长逐渐下降;虾、尘螨组合的过敏原阳性率则随着年龄的增长而逐渐上升,猫毛的特异性过敏原阳性率以<3岁组最高,霉菌组合阳性率较高的年龄组为3~6岁组,差异均有统计学意义(P<0.05),不同年龄组蟹、花生、屋尘过敏原阳性率比较,差异无统计学意义(P>0.05);<3岁组中,男患儿牛奶、尘螨、猫毛、狗上皮过敏原阳性率高于女患儿,差异有统计学意义(P<0.05),不同性别患儿鸡蛋白、虾、蟹、花生、牛肉、屋尘、树组合、霉菌组合过敏原阳性率比较,差异无统计学意义(P>0.05)。结论多数AD患儿血清总IgE和特异性IgE的趋势为升高,检测特异性IgE可帮助发现AD患儿过敏原,该地区食物性过�Objective To analyze the characteristics of serum specific allergens in children with atopic dermatitis(AD)to provide reference for the prevention of this condition in hospitals.Methods This study included 685 children diagnosed with AD at the 988th Hospital of Joint Logistics Support Force of CPLA from April 2021 to March 2024 for retrospective analysis.Serum total immunoglobulin E(TIgE)and serum specific immunoglobulin E(sIgE)were measured in all patients.The results of serum TIgE and sIgE were analyzed,and the positive rates of allergens in different age groups and genders were compared.Results Among 685 AD patients,490 patients showed positive results for serum TIgE,yielding a positivity rate of 71.53%.There were 471 patients with serum TIgE levels above 200 kU/L(68.76%),and 19 patients with levels between 100 and 200 kU/L(2.77%).The number of patients with positive serum sIgE results was 428,with a positivity rate of 62.48%.Among these,232 were male(33.87%),and 196 were female(28.61%).The numbers of patients with positive results for 1,2,3,and 4 or more allergens were 178,131,98,and 80,respectively.High positivity rates for food allergens included milk and egg white,while dust mites and cat dander had high positivity rates among inhaled allergens.Positive rates of specific allergens such as egg white,milk,shrimp,dust mites,cat dander,and mold varied significantly across different age groups(P<0.05),with egg white and milk showing a gradual decrease in positivity rates as age increased,whereas the positivity rates for shrimp and dust mites increased with age.The highest positivity rate for cat dander was observed in the group under 3 years old,and the highest rate for mold was found in the 3~6 years age group,with statistical significance(P<0.05).In the group of children aged under 3 years,the positive rates of allergens for milk,dust mites,cat dander,and dog dander were higher in male patients than in female patients,with the difference being statistically significant(P<0.05).However,when comparing the posit
