MDT干预在骨科创伤后应激障碍患者中的应用

摘　　要：目的探讨多学科综合治疗(MDT)在骨科创伤后应激障碍(PTSD)患者中的应用效果。方法选取60例骨科PTSD患者,按随机数字表法分为对照组(n=30)和试验组(n=30)。对照组采取常规护理干预,试验组采取MDT干预,2组均干预2周。比较2组干预后的精神症状(注意障碍、情感障碍、意志行为障碍和记忆障碍)、PTSD自评量表(PTSD-SS)评分及焦虑、抑郁程度[采用汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD-17)评定]。结果干预后,试验组精神症状发生率、患者PTSD-SS评分及焦虑、抑郁程度均低于对照组(P<0.05或P<0.001)。结论MDT干预可改善骨科PTSD患者的精神症状、创伤程度和心理状态。Objective To explore clinical efficacy of multidisciplinary comprehensive treatment(MDT)in orthopedic patients with post-traumatic stress disorder(PTSD).Methods 60 orthopedic patients with PTSD were selected and randomly divided into a control group(n=30)and an experimental group(n=30)using a random number table.The control group received routine nursing,whereas the experimental group underwent MDT,both groups intervened for 2 weeks.Psychiatric symptoms(attention disorders,emotional disorders,volitional behavior disorders,and memory disorders)and scores on PTSD Selfrating Scale(PTSD-SS)were compared between the 2 groups after intervention,along with the degree of anxiety and depression assessed using Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD-17).Results After intervention,the incidence of psychiatric symptoms,the score of PTSD-SS,and the degree of anxiety and depression in the experimental group were all lower than those in the control group(P<0.05 or P<0.001).Conclusion MDT can potentially improve psychiatric symptoms,degree of trauma and psychological status in orthopedic patients with PTSD.
