摘　　要：目的:探讨PDCA循环管理在第二类精神药品使用管理中的应用效果。方法:将2022年7—12月广州医科大学附属妇女儿童医疗中心门急诊开具的第二类精神药品处方4710份作为管理前组(实施常规管理),将2023年1—6月门急诊开具的第二类精神药品处方5060份开具的作为管理后组(实施PDCA循环管理)。比较两组管理效果。结果:管理后组不合理处方占比低于管理前组(P<0.05)。两组不合理处方类型主要为单张处方超量开具、无适应证用药、用法用量不适宜、适应证不适宜;管理后组单张处方超量开具、适应证不适宜占比均低于管理前组(P<0.05);两组无适应证用药,用法用量不适宜,开具处方未写临床诊断或临床诊断书写不全,重复取药,处方的前记、正文、后记内容缺项且书写不规范或字迹难以辨认,遴选的药品不适宜,无正当理由为同一患者同时开具>2种药理作用相同药物占比比较,差异无统计学意义(P>0.05)。结论:PDCA循环管理在第二类精神药品使用管理中的应用效果显著,能够减少不合理处方,保障患者用药安全性。Objective:To explore the application effect of PDCA cycle management in the use management of second-class psychotropic drugs.Methods:A total of 4710 prescriptions for second-class psychotropic drugs prescribed by the outpatient and emergency departments of the Women and Children's Medical Center Affiliated with Guangzhou Medical University from July 2022 to December 2022 were included as the pre-management group(routine management was implemented).A total of 5060 prescriptions for second-class psychotropic drugs prescribed by the outpatient and emergency departments from January 2023 to June 2023 were included as the post-management group(PDCA cycle management was implemented).The management effects were compared between the two groups.Results:The proportion of irrational prescription in the post-management group was lower than that in the pre-management group(P<0.05).The types of irrational prescriptions in two groups were mainly overdose issuance of a single prescription,medication without indications,inappropriate usage and dosage,and inappropriate indications.The proportions of overdose issuance of a single prescription and inappropriate indications in the post-management group were lower than those in the pre-management group(P<0.05).There was no significant difference in the proportions of medication without indications,inappropriate usage and dosage,absent or incomplete clinical diagnosis in prescriptions,repeated fetching drug,missing items in the preface,text and afterword of prescriptions with non-standardized writing or illegible handwriting,inappropriate selection of drugs,and prescribing>2 drugs with the same pharmacological effect for the same patient without legitimate reason between two groups(P>0.05).Conclusion:The application effect of PDCA cycle management in the use management of second-class psychotropic drugs is significant,which can reduce irrational prescriptions and ensure the medication safety of patients.
