等离子射频消融术联合鼻中隔成形术治疗鼻中隔偏曲伴代偿性下鼻甲肥大的效果分析

摘　　要：目的:探讨等离子射频消融术联合鼻中隔成形术治疗鼻中隔偏曲伴代偿性下鼻甲肥大的临床效果。方法:选取2021年3月—2024年3月苏州永鼎医院收治的鼻中隔偏曲伴代偿性下鼻甲肥大患者93例作为研究对象,随机分为消融组(n=46)和骨折外移组(n=47)。骨折外移组给予鼻中隔成形术联合下鼻甲骨折外移术,消融组给予鼻中隔成形术联合等离子射频消融术。比较两组鼻功能、鼻疼痛情况、疗效、并发症发生率。结果:术前,两组鼻纤毛输送率、鼻阻力值、鼻疼痛评分比较,差异无统计学意义(P>0.05)。术后3个月,两组鼻纤毛输送率、鼻阻力值、鼻疼痛评分低于术前(P<0.05),其中消融组鼻纤毛输送率高于骨折外移组,鼻阻力值、鼻疼痛评分低于骨折外移组(P<0.05)。消融组治疗总有效率高于骨折外移组(P=0.049)。两组并发症发生率比较,差异无统计学意义(P=0.664)。结论:等离子射频消融术联合鼻中隔成形术治疗鼻中隔偏曲伴代偿性下鼻甲肥大的效果显著,可改善患者鼻功能,减轻术后疼痛,且手术安全性好。Objective:To explore the clinical effect of plasma radiofrequency ablation combined with septoplasty in treatment of deviated nasal septum with compensatory inferior turbinate hypertrophy.Methods:A total of 93 patients with deviated nasal sep⁃tum and compensatory inferior turbinate hypertrophy who were admitted to Suzhou Yongding Hospital from March 2021 to March 2024 were selected as the study subjects.They were randomly divided into the ablation group(n=46)and the fracture displace⁃ment group(n=47).The fracture displacement group received septoplasty and inferior turbinate fracture displacement,and the ab⁃lation group received septoplasty and plasma radiofrequency ablation.The nasal function,nasal pain,efficacy and complication in⁃cidence were compared between the two groups.Results:Before operation,there was no significant difference in nasal ciliary trans⁃port rate,nasal resistance value and nasal pain score between the two groups(P>0.05).At 3 months after operation,the nasal cili⁃ary transport rate,nasal resistance value and nasal pain score in the two groups were lower than those before operation(P<0.05),and the nasal ciliary transport rate in the ablation group was higher than that in the fracture displacement group,and the nasal re⁃sistance value and nasal pain score in the ablation group were lower than those in the fracture displacement group(P<0.05).The total effective rate of treatment in the ablation group was higher than that in the fracture displacement group(P=0.049).There was no significant difference in the incidence of complications between the two groups(P=0.664).Conclusion:Plasma radiofrequency ablation combined with septoplasty has a significant effect in the treatment of deviated nasal septum with compensatory inferior turbinate hypertrophy,and can improve nasal function and relieve postoperative pain,and the operation safety is good.
